18 platforms checked against their own terms. None of the AI app-builders sign a BAA. Lovable, Bolt.new, Base44, Replit and v0 all sit at not-applicable: a BAA is a property of the infrastructure holding the data, not the tool writing the code. The layer underneath signs. Firebase and the big three cloud model APIs by default; Vercel, Supabase, OpenAI and Anthropic on a paid tier; Netlify, Mistral, Clerk and Resend at Enterprise only. No BAA means no PHI on that vendor, on any plan, at any price. Every row carries a verified date and a watcher that re-checks for drift. Which row surprised you? cliniciansthatcode.com/guide…

question for @herdrdev and @NousResearch : How can i keep track of the kanban progress in Herdr? Can't get a decent result apart from a small script that i've open in a tab.

Ozempic and Mounjaro beat Metformin at preventing prediabetes from progressing to diabetes, and they also seem to protect the heart. In health records from 256,492 people with prediabetes and obesity, people prescribed a GLP-1 drug had 31% less type 2 diabetes over about 3 years than those prescribed Metformin, along with 15% fewer heart attacks and 20% fewer strokes. Metformin still has solid evidence behind it. In 2002, it cut new diabetes by 31% compared with a placebo. The death number is the one to treat carefully. Deaths were 41% lower here, but Wegovy, a higher dose of Ozempic, cut heart events by 20% in a trial of 17,604 people. That gap probably reflects who gets these drugs, not a clean head-to-head result. The diabetes finding is backed by trial data. Over 3 years on Mounjaro, 1.3% of people with prediabetes developed diabetes, versus 13.3% on placebo. Bottom line: if your blood sugar is creeping up, ask your doctor whether a GLP-1 drug or Metformin makes sense for you.

@tijd

Would love some advice on MacWhisper @jordibruin regarding AI-powered dictation. I currently have a local model (Gemma 4) doing this, but it is rather slow. I experimented with a couple of other models, both cloud models and local models, but it's still not fast enough. What is your recommendation to have good quality and very fast execution for the AI-powered dictation?

Three specialised agents instead of one model: that is the architecture a new Nature paper proposes for personal health. One analyses your wearable data statistically, one grounds answers in medical evidence, one coaches behaviour change. Most consumer health chatbots blur all three into one confident voice. The tension: every handoff between agents is a place for error to hide. A coach motivating you on a flawed analysis is worse than no coach. Who audits the seams? nature.com/articles/s41586-0…

For twenty years, every fix a nurse needed in her software went through me. She explained what broke at 3am; I translated it into code; days later she got a fix that was usually right. The insight was always hers. The bottleneck was me. That bottleneck has fallen. Writing code was maybe 5 to 10% of making clinical software real, but it was the 10% that stood in front of everything else. The compliance, testing and maintenance are still there. You can now reach them in days instead of years. What would your night shift build first? cliniciansthatcode.com/essay…

81% of organisations say AI has made their decisions faster. Only 25% are using it to deeply transform the business. The technology is advancing at extraordinary speed, but using more AI at any cost isn't the same as using it wisely. Speed is becoming common. Redesigning the work around it is still rare. Another number from the same Deloitte report: 71% track their total AI spend, only 44% track the cost per unit. Most can see the bill, few can see what it buys. If AI only makes your current process faster, what have you actually changed? deloitte.com/us/en/what-we-d…

“Other partners, notably the Flemish socialists Vooruit, continue to prioritise additional taxes and social spending growth. They are effectively protecting the same unreformed spending trajectories that contributed to the present situation.”
Belgium needs a credible multi-year path to stabilise the numbers under EU rules. Years of postponed structural reforms under previous governments (on pensions, healthcare cost growth, public administration and labour-market participation) have left less room for manoeuvre today. That is the inherited problem. The current coalition under Prime Minister @Bart_DeWever has been negotiating roughly €10 billion in measures. His @de_NVA and the liberal MR have pushed for the bulk of the effort through genuine spending restraint. Other partners, notably the Flemish socialists Vooruit, continue to prioritise additional taxes and social spending growth. They are effectively protecting the same unreformed spending trajectories that contributed to the present situation. This is an economic own-goal. A substantial empirical literature (Alesina, Favero, Giavazzi and IMF analysis) shows that spending-based consolidations are systematically less damaging to growth than tax-based ones. Tax increases tend to produce larger and more persistent output losses, while targeted expenditure restraint, especially where it improves efficiency in benefits and the public wage bill, has smaller short-run effects and better long-run debt dynamics. In a high-debt eurozone country, relying mainly on new taxes while leaving spending growth largely untouched raises the risk of higher borrowing costs and weaker growth without solving the underlying problem. IMF work on Belgium has long emphasised the need to rationalise social benefits and administration costs rather than lean primarily on additional taxation. The partners blocking a balanced, expenditure-heavy package are choosing short-term political comfort over a path that economic evidence shows is less harmful. Supporting Bart De Wever’s insistence on real spending control is support for a more growth-friendly and sustainable adjustment, one that keeps the decisions in Belgian hands instead of letting the numbers deteriorate further through inaction. It insane how irresponsible the coalition partners of NV and MR have been. If Belgium does not act, the EU’s Excessive Deficit Procedure will. External conditionality is rarely gentle and never tailored to domestic political comfort. The parties that refused to reform earlier and doing this again, are largely to blame for the current bind. Finishing the job with real expenditure control keeps the decisions in Belgian hands. The alternative will definitely not be nicer.

Uber delivery workers in France are reportedly running several phones, declining low offers until the algorithm raises its price. It is eBay shill bidding in reverse. The logic is neat: dynamic pricing treats each offer as a probe of what you will accept, so workers feed it a false reservation price. The tactic is banned, and anyone caught loses future work. Expect this to spread to consumers too, through AI agents built for deal hunting. When both sides run optimisers against each other, who actually sets the price?

A 500-bed hospital spends around EUR 2.5 million a year reconciling patient data that lives in a dozen systems. An MRI in radiology, bloods in the lab, medication notes in an ageing record, lifestyle metrics in a phone that never pairs. Every AI pilot opens with the same cry: get quality data in one place. The Model Context Protocol is the most interesting answer yet. Think USB-C for healthcare data: build once to a single port, and any compliant AI application can pull the context it needs, meaning intact. Where would one context port change your pipeline first? hyperadvancer.com/blog/from-…
