Looks like I’m getting a reputation- thanks @matthew608b- very clever! In all fairness- sometimes bicarb is the answer- sometimes. @DenverEMed @CUEmergency
Hot off the press @NEJM
“Prehospital Extracorporeal Life Support in Refractory Out-of-Hospital Cardiac Arrest”. Dr. Reis Miranda presents the results of the OnScene Trial at #ReSS26. 2x better survival in helicopter assisted ECPR!! @DYannopoulos @Dezfulian_CCM @AshPanchalMD @AHAScience @American_Heart
Dr. Yannopoulos presents the Dickinson Richards @3CPRCouncil Memorial Lecture on his groundbreaking work in ECPR in OHCA and future directions to improve survival after OHCA. Congratulations @DYannopoulos
#ReSS26 @AHAScience @American_Heart
And we’re off! #ReSS26 begins with a deep dive into the ICECAP Trial and subsequent ancillary studies and secondary analyses. Tremendous US-based work in over 1100 OHCA patients. @AHAScience @3CPRCouncil @willmeurer @AshPanchalMD @Dezfulian_CCM
Long term outcomes in pediatric cardiac arrest recovery are challenging as children potentially “grow into their deficit”. Important work ongoing in pediatric ICECAP extended (PIE). @KennedyKrieger @BethSlomine #ReSS26
Innovative pediatric outcomes to measure recovery after pediatric OHCA in the pediatric-ICECAP trial currently enrolling! @AlexisTopjian #ReSS26
Dr. Callaway gives an update on the #POSTICECAP study exploring functional and psychological outcomes after survival fromOHCA. #ReSS26

#PRECICECAP study exploring cardiac arrest phenotypes reports separation of cohort with promising findings to support future work. Stay tuned for exciting results in the spring! @AHAScience #ReSS26 @KarenHirschMD
HIBI secondary analysis of ICECAP shows higher quality temperature control (shorter time to devise and use of NMB) is associated with improved outcome. #ReSS26 @AHAScience

ASPIRED #ESCCongress
2,233 pts with unexplained syncope presenting to ER (48% women 👏🏽🌟) → immediate 14-day ECG monitor vs usual care
No ↓ recurrent syncope at 1 yr, BUT:
📈 Arrhythmia detection: 22% vs 9%
📉 Mortality: 1.5% vs 2.9%
Early monitoring finds actionable arrhythmias

Nobody teaches you how to lead a room during a resuscitation.
You don't become calm because you aren't scared. You become calm because everyone else needs you to be.

Out-of-hospital cardiac arrest can look devastating early.
The neurologic exam may be poor. The CT may show edema. Families understandably want answers immediately.
But one of the most important principles in post-arrest care is avoiding premature neuroprognostication.
