Robert and I are thrilled that our gift of over 2 million shares of SpaceX stock will be distributed to children’s Trump accounts in December!
Our initial gift (shares transferring to escrow as we speak) will go to 11 to 17-year-olds who live in zip codes with median family income (MFI) of $57,000 or less, and to those who live in zip codes with too few residents for an MFI to be reported in the census. These families should receive a notice by Monday.
In a few weeks, an additional gift will be finalized for eligible 11 to 17-year-old children in military families serving overseas, children in foster care, and more children whose MFI is $90,000 or less living in counties near our home in the great state of Texas.
Thank you to the incredible team @SpaceX, @Treasury, @IRS, @DellFdn, @GreaterHorizons, @InvestAmerica24, and the legal firms of Morgan Lewis and Gibson Dunn for working over the past few months to make this happen.
We are deeply grateful for the groundbreaking work that Brad Gerstner @altcap and @MichaelDell have done for the long-term financial security for millions of kids.
God Bless America!

Obamacare premiums jumping 13% next year means paying $1,200 a month just for the privilege of paying cash for every doctor visit until you hit a $9,000 deductible.
That isn't protection. It's an annual subscription fee to an insurance company that still denies your claims.

Independent physicians simply cannot compete with the lobbying of the other stakeholders and the AMA’s advocacy for doctors is undermined by the CPT coding business gifted from insurers and hospitals.
For 2027, CMS is proposing another reduction in physician payments while increasing hospital facility payments.
Over the last 20-plus years, physician payments have lost ground to inflation. Hospital outpatient payments, meanwhile, receive regular inflation-based updates.
For a routine level 3 visit, the proposed changes are:
Independent office: about $95 → $93.
On-campus hospital clinic: about $193 → $200–$204, including physician and facility payments.
In my office, that $93 has to cover my time, staff, rent, equipment, EMR and billing.
The physician payment in the hospital falls too. The increase comes from the separate facility fee.
Independent practices face rising overhead without a separate facility payment to offset the cut. That makes independent practice harder to sustain and encourages routine care to move into hospitals, where the total payment is more than twice as much.
I completely agree here. Fear is a great motivator. Set a goal. Sign up. Get training.
The single fastest way to improve your entire life is to sign up for a physical side quest. Race. Competition. Adventure. Whatever. Your day becomes more structured with the training. You sleep deeper because you're tired. You eat cleaner. You look better. You feel better. Everything falls into alignment. It's a psychological process disguised as a physical one.

Women don't need a doctor's referral to get a routine mammogram under federal rules. Yet some imaging centers still refuse to schedule patients without one.
Breast cancer screening should be easier than this. There's no reason to make women jump through unnecessary hoops.

There are ~20 legislative days in the House lame duck session.
Per our regulatory advisors, during the past ~4-5 lame duck sessions, over 130 bills were passed.
While no math major, if you do the math, these bills will have an abbreviated time period for consideration.
So, now is the time to contact your member of Congress on HR 4710.

For more than a decade I have told our clients, physician owned hospitals and ASCs that legislation for healthcare “passes” during lame duck.
Mr. Ed Gaines reports IDR and No surprises Act will be “discussed” during lame duck…

The single most significant external factor in determining the decline in one's mental health is exposure to the modern medical mental health system.
I'll die on that hill.

but there is a massive shift happening in human biology right now.
it's sitting in press releases and almost nobody reads them
the last 6 months alone:
- a once-daily pill roughly doubled median survival in metastatic pancreatic cancer (13.2 vs 6.6 months)
- a one-time gene editing infusion cut hereditary angioedema attacks 87%, and 62% of patients stayed attack-free and off all other prevention for 6 months
- a one-time gene edit for cholesterol dropped LDL up to 62%, and it held for over a year
- a personalized mrna vaccine beat keytruda alone in a 1,137 patient melanoma phase 3
- retatrutide hit ~28% weight loss in a phase 3
- an ai-designed drug entered phase 3
- a pill that regrows hair passed its second phase 3
- an immunotherapy that could let some rectal cancer patients skip surgery, chemo and radiation just got fda approved
- a man lived 271 days off dialysis on a pig kidney
- human cells were caught passing chromosome-sized chunks of DNA to their neighbors, and it changes how the neighbors behave
we are on the verge of an era where the human body is a software problem and the update cycle gets shorter everyday
stick around, it's going to get weird

Every man made product was someone’s idea first. Why not you? Why can’t you be the person to come up with the next great product or service.
And if it fails. So what. You only have to be right one time

For the first time, we're hearing about previously undisclosed connections between Palantir and some of America's biggest health insurers: Aetna, Humana, Cigna and Blue Shield of California.
A new report from the Center for Health & Democracy Education Fund examines documents obtained by the Electronic Frontier Foundation that claim Palantir's technology is already being used by insurers for functions including "decision support."

People often ask me why I advocate so strongly for independent physician practice. I am an employed doctor, after all.
Well, this is why👇
Employed docs are just widget makers. Produce RVU, click the boxes, follow the protocols.
They’ll replace us as soon as they can get away with it.
A thriving independent physician sector keep us all safer.
Anyone not surprised pediatrics is the first to go? Silly employee docs, your heads are the next to roll.

The second healthcare store we built was @GeneralMedicine.
So it only felt appropriate that the third would be @Walmart.
