Thank you, Dr. Mussa for joining us for this informative webinar! If you or your loved one is living with a type B (descending) arotic dissection, we strongly recommend watching the recording. Lots of great, data backed recommendations on how to live a full and active life!
Thank you @JohnRitterFdn for being such patient advocate, supporting science and spreading hope especially among those who may feel alone

Today, Thursday August 13th, @JohnRitterFdn arranges a webinar around Aortic Dissection. The people behind this are Professor @UnTBAD et al. 7PM CT Dr Sherene Shalhub speaks on Type B Aortic Dissections. Go to the Ritter Foundation for more info about how to join in. #aortaEd
The search for answers is not only the US. @EarnestTrial is one of three global effort to move away from opinions to data @SUNDAYtrial #IMPROVE-AD @VascularSVS
Aortic dissection is a life-threatening emergency where every minute matters. In this interview, Dr. Firas Mussa and I discuss the key signs and symptoms clinicians should recognize, why early diagnosis is so critical, and how the IMPROVE-AD trial aims to advance the care of patients with acute aortic dissection.

A new review by Firas F. Mussa, MD, and Panos Kougias, MD (@KougiasP), describes the current management of type B aortic dissection, including medical therapy and thoracic endovascular aortic repair.

ALT Panel A: The mechanisms by which an aortic dissection may lead to end-organ ischemia, including static obstruction, in which the side branch is occluded completely by the dissection flap, with or without thrombus. Panel B: The axial and sagittal computed tomography (CT) images showing malperfusion of the celiac artery caused by an aortic dissection. Panel C: The axial and sagittal CT images showing malperfusion of the superior mesenteric artery caused by an aortic dissection.

A fatal condition that’s is poorly understood. Even the @nytimes got it wrong!!!!
Patients deserve better science

Thanks @VascularSVS @DrBabakAbai for supporting science and generating the evidence for unTBAD
On this day, 4/26, Lucille Ball died of ruptured AAA. A week prior she had a type A repair. Were they linked? Genetics? How would that be different today? 37 years later and we still looking for answers #IMPROVE-AD
👇is another one on why we need a trial. Glad to see @JVascSurg making a turn back to good science

👇for all those who push to TEVAR without data. Maybe time for @VascularSVS to push for more science and less social reform!
Excessive overlap of stent grafts significantly increases the complexity of subsequent open surgery.
In our experience, removal of an overwrapped stent graft invariably damages surgical instruments; scissors used for cutting the stent graft consistently lose sharpness and must be replaced immediately after the procedure. This clearly reflects the extreme mechanical burden imposed on open conversion surgery.
We have treated more than 20 cases of esophageal fistula following TEVAR, and based on this experience, we firmly believe that esophagectomy is mandatory once an esophageal fistula is diagnosed.
In our series of these highly complex salvage operations, patients who did not undergo simultaneous esophagectomy demonstrated significantly worse long-term outcomes compared with those who underwent definitive esophageal resection.
Therefore, we routinely perform aggressive omental flap interposition, filling the infected field as much as possible, in combination with esophageal reconstruction using a gastric tube. We consider this strategy essential for infection control and long-term survival.
From a broader perspective, we strongly caution that indiscriminate use of TEVAR without careful long-term consideration will inevitably increase the frequency of extremely complex and high-risk open salvage surgeries. What is initially perceived as a minimally invasive solution may ultimately result in a far more invasive and hazardous surgical burden.
At at time when funding at at all time low (IMPOSSIBLE), #IMPROVE-AD needs everyone to enroll. Once in a lifetime chance and we are here to make it happen 🙏 @KougiasP @SUNDAYtrial
The @NIH has approved funding for the WARRIORS international randomized trial comparing endovascular aneurysm repair (EVAR) with watchful waiting in women with small abdominal aortic aneurysms (AAAs) measuring 4–5.5 centimeters.

Patient input, supported by TADCT, has been vital to the design of the @EarnestTrial. Prof Bicknell speaks with Steve Perryman MBE about what patients need to know before taking part. If you’ve experienced an aortic dissection or know someone who has, support is available. tadct.org/EARNEST
