🚨 ARTO long-term update in oligometastatic mCRPC:
SBRT to all mets + abi/ADT vs abi/ADT alone.
@TheLancetOncol
👥 157 pts
🎯 ≤3 bone/nodal mets
💊 Abiraterone + ADT ± MDT/SBRT
⏱️ Median follow-up: 53 mo
Results:
✅ OS: NR vs 50 mo
✅ HR 0.55, p=0.021
✅ bPFS: 44 vs 18 mo
✅ rPFS: 44 vs 17 mo
✅ PC-specific survival improved
✅ No major toxicity signal from adding SBRT
Big signal: MDT may be more than “delay next therapy” in oligometastatic CRPC.
Caveat: phase 2, unplanned OS analysis, not originally powered for OS.
Still: hard to ignore. 🔥

Interesting study demonstrating equivalent long-term outcomes with treatment continuation vs interruption in exceptional responders to PARPi in PS-ROC. May support strategy of PARPi interruption in exceptional responders in met PrCa as well.

DE-ESCALATE (@EORTC): A pragmatic trial to revisit intermittent ADT in mHSPC in the era of new ARPIs @urotoday #ASCO26
🌟n=1,600
🌟Key elig criteria: mHSPC pts, PSA ≤ 0.2 at 6-12 mos after Rx start
🌟2:1 randomization to cMAB vs iMAB
🌟Co-primary EPs: (i) feasibility (ii) overall survival
🌟Status: May 1, 2026: 211 pts enrolled

CLEARED: Comeback from long course ADT with relugolix and daro in hormone-sensitive PCa @AtishChoudhury @urotoday #ASCO26
🌟Rel + Daro: safe, well-tolerated during first 6 cycles -- PK parameters similar to historical mono Rel
🌟Pts will be followed after end of Rx for 1 EP of T recovery

How early is too early for lutetium PSMA in metastatic prostate cancer?
The data is evolving and comfort with the therapy is growing, but sequencing remains unsettled.
Should lutetium PSMA move forward in mCRPC while patients are fitter and disease burden may be lower? Or is its impact strongest when reserved for later lines, after AR targeted therapy and chemotherapy?
At #OLNGreatDebates, @kai_tsao, MD, MS, and @AtishChoudhury MD, PhD take opposing positions on when this therapy truly delivers the most value.
Hear both sides. Then decide where you stand.
