Congrats to @PaulKaranicolas and @CHPBAsurg for this amazing achievement and important paper!
Thanks to all the amazing patients, surgeons, and researchers who made this trial possible.
TXA did not reduce bleeding or blood transfusion but increased periop complications after liver resection.

Does the benefit of #surgery over #endoscopy for symptomatic wide-duct (PD >5mm) #chronic #pancreatitis remain in 8-year follow-up?
Yes! And benefits of surgery getting bigger.
💎 very satisfied: 71% vs 33%
💎 would recommend: 87% vs 47%
💎 worse complete pain relief with secondary surgery after failed endoscopy vs primary (upfront) surgery: 55% vs 12%
💎 even when “complete ductal clearance” obtained by endoscopy; still superior pain scores with primary surgery: 2.9 vs 4.8
So, the “lets try endoscopy first” or “I can clear this duct endoscopically” no longer holds….

Is it safe to stop placing a drain after distal pancreatectomy?
➡️Today #PANDORINA non-inferiority trial 🇳🇱🇮🇹 @LancetGastroHep
✅ Yes! Not only safe, but improves overall morbidity and POPF B/C rate!

🚨PANDORINA, a binational 🇳🇱🇮🇹RCT of drain 🆚 no drain in distal pancreatectomy out in @LancetGastroHep !
✅Major morbidity 20% vs 15%
✅POPF 27% vs 12%
➡️A no-drain policy is safe and reduced POPF and should be the new standard approach
