What a privilege to implant @MedStarWHC's first @AbbottCardio Aveir VR Mini #ShortKing via the right IJ access in a patient with an IVC filter. Incredible maneuverability and handling with the new #smol profile. First Aveir Mini VR Right IJ in the world 🤪 #LeadlessFirst #EPeeps

Jealous of my amazing partner Rajiv Kabadi @spurs21rak who made this challenging #Aveir helix-fixation leadless pacemaker implant through an @EdwardsLifesci Evoque #TTVR look easy 🤩 LPM avoids traversing and potentially hindering valve coaptation 😍 #LeadlessFirst @EPeeps_Bot

Abdrakhmanov et al. report leadless pacemaker implantation at 17 weeks’ gestation with an estimated fetal dose of just 1.4 mGy and a healthy infant delivered at term.
Careful radiation optimization + multidisciplinary planning matter. #EPeeps #HRO2GlobalVoices

Please join us at arrhythmiaacademy.com for our program Advancing Physiologic Defibrillation Therapy with a Dedicated CSP ICD Lead on October 27th! I'm honored to have the privilege of discussing this incredible lead! Recynchronize and protect 😍
I hope to meet lots of fellow #LeadlessFirst enthusiasts tonight and tomorrow at the @AbbottCardio #AVEIR Leadless Pacemaker Summit in Dallas! Be there or be transvenous! 😂 #EPeeps @GTCardFellows

Short vid from experiments done Sunday examining endocardial damage with different retrieval methods after entanglement.
This is typical of unsupported traction on the lead.
Huge amount of endocardial delamination and damage.

*⃣Percutaneous Retrieval of Embolized Atrial Leadless Pacemakers: Early Multicenter Experience @rdschaller @MGKatz036 Now online in @hrs_journal
➡️The first multicenter case series of AVEIR AR embolization reports 6 cases, all occurring during the early commercial experience (median 5 prior implants per center, range 1–10).
➡️The estimated embolization rate was 3.6%.
➡️Every embolized device had minimal/ absent COI at implant.
➡️All devices embolized with the fixation helix oriented proximally, suggesting the device tends to migrate by gravity while maintaining this orientation within the PA tree (5 cases) and Right iliac vein (1 case).
➡️All devices were successfully retrieved percutaneously using common extraction tools (the dedicated AVEIR retrieval sheath was too stiff).
🌟These findings complement MAUDE reports of 22 dislodgements, with 21 retrieved (1 open surgical).
🧑🎓Clinical takeaway:
➡️Percutaneous retrieval of embolized AVEIR AR devices appears feasible and relatively safe in experienced hands.
➡️Success may depend on duration of embolization. Delayed recognition & delayed transfer to extraction centers may permit progressive distal migration into the PA tree, potentially increasing procedural challenge.

PFA- No benefit for PWI and trend of more recurrence with AFL/AT.
Still lacking evidence for PWI in persistent AF.
