If we don’t refer uncomplicated hypothyroidism to endocrinology, or refer mild hypertension to cardiology, why are we referring vasomotor symptoms out?
Menopause care belongs in primary care.
Labels are changing, but the responsibility remains on the clinician to provide access and personalize the dose, and route to the patient needs and history.

Pharmacologically, progestins do the real work in contraception, not estrogen. Finding the right contraception fit considers progestin type, generational differences and formulation.

The message to women is often polarized. Hormonal #contraception = bad
#Menopause hormones therapy = good.
That’s negativity bias.
For some, contraceptives fail, but for the right woman, HC can be life-changing.

Same hormones. Very different roles. Contraceptives suppress ovulation with high doses of "synthetic" hormones, but MHT aims to replace missing hormones at physiologic levels. It is the indication, history, dose, route, and life stage that determines risk, not just hormone type.
Dose ≠ effect! Early COCs used >50 mcg ethinyl estradiol, but modern COCs use 20–35 mcg and remain effective because, pharmacologically, EE is highly potent. MHT often uses higher milligram doses because the estrogens used are weaker. Clearly, potency and clinical intent matter.
267 #FDA-approved contraceptives contain lactose. Some hormone therapies include peanuts, shellac, or FD&C Red No. 40.
