Fun session at the track. I really like late summer early fall running. Today was 10x600m at threshold with 200m jog rest and 3k warmup and cooldown.
Somewhere in here I ran a sub 20 5k and 40:19 10k.

continuing empiric vancomycin for >48 hours without *any* evidence of MRSA (no PCR, culture, or gram stain data to support the presence of MRSA) should be a federal crime.

ICYMI - Guidelines recommend catheter ablation for symptom relief in patients with atrial fibrillation. In this study, catheter ablation did not demonstrate superiority over a sham procedure for improving atrial fibrillation-related quality of life at 6 months: spkl.io/60117r8rZ

Great work @DrToddLee and team. It’s a fascinating debate about what’s the “best” or “worst” beta lactam. This certainly provides superior information to small observational studies.
Finally it's published!
🆕🔥🟢Systematic Review and Bayesian Meta-Analysis @DrToddLee
What is the association of cefepime with all-cause mortality compared with other β-lactam antibiotics?
This systematic review and bayesian meta-analysis found that cefepime was associated with higher odds of all-cause mortality compared with other β-lactams Meta-analysis of published peer-reviewed RCTs (73 trials, 15 411 patients) showed that cefepime was associated with a 98.6% posterior probability of higher mortality compared with other β-lactams (OR, 1.17; 95% credible interval, 1.02-1.34). Posterior distributions generally favored higher odds of mortality across all comparator β-lactams, across all clinical indications, and with doses of 2 g or more every 12 hours. #idxposts jamanetwork.com/journals/jam…

This is one of the reasons I have been going on about this cefepime/pip-tazo story. The weight of RCT evidence suggests pip-tazo is a *superior* agent. Yet a bunch of observational studies suggest some anti-anaerobic activity is harmful. It's pretty clear what we should trust!
Finally it's published!

This was a multi year masterpiece written by Mayo clinic's new attending @ZahraNaurish while she was finishing training with @DrEmilyMcD and me.
I would suggest that guidance/guidelines should reflect on this data and downgrade the love of cefepime if they're evidence based
Finally it's published!
