Guiding principles for me & (I hope) other MDs:
-All patients we touch get high quality care.
-Care decisions arise from an informed, empathetic doctor/patient relationship.
-In gray/undecided areas, the best path is usually the less intrusive/invasive/expensive path.

LBB Area Pacing vs RV Pacing for AV Block and Preserved Left Ventricular Function:
The #LEAP-Block Trial
📍499 patients with #LVEF ≥50% randomized to LBBAP vs #RVP across 14 Chinese hospitals.
📍 Primary endpoint: 2.4% vs 12.5% favoring #LBBAP (P=0.003).
📍 HF events: 3.4% vs 11.3% with LBBAP vs RVP.
📍 LBBAP ⬇️ the 24-month primary endpoint risk (HR 0.37, 95% CI 0.19–0.73), with no significant differences in mortality or device complications.

My hot take on the iPhone Duo:
Luxury iPhone for folks who don't take their iPhone too seriously. Most reviewers take their iPhone too seriously.

Proud of our TRIAD fellow Michel Abou Khalil @mabou_khalil for this important work in JCE!
In nearly 19,000 patients undergoing first ablation for paroxysmal #AFib, adding lesions beyond PVI was associated with more recurrence, repeat ablation, and atypical flutter.
An observational study, but an important reminder: more ablation does not necessarily mean better outcomes. Defining when—and for whom—to go beyond PVI remains critical.

Out now in JCE: is more ablation better in paroxysmal AF?
18,891 pts, 27% got PVI+ at first ablation. After matching, PVI+ led to:
↑ Recurrence (HR 1.28)
↑ Redo ablation (HR 1.99)
↑ Atypical flutter (HR 2.63)
↑ Persistent AF (HR 1.37)

Interesting piece. One question from the EP side: if interventional HF encompasses TEER, MCS, CCM, etc., shouldn’t CRT/CSP—one of the great success stories in HF—have a more central place? And if @HFSA @SCAI will co-own this emerging field, shouldn’t EP @HRSonline have a seat at the table?
Excellent statement on the journey so far and path forward toward interventional heart failure (IHF) as a discipline. Worth a read.

My favorite Adidas cap, veteran of countless Warped and Lollas had it's last rodeo at @RiotFest 2026. Lost forever in the Bad Religion circle pit. RIP

@ShivdevRao has talked about using Ambient AI to “scale empathy.” We can make the @ClevelandClinic Empathy video come to life. 👇

@AbridgeHQ sidebar does more than CDS-
Inspired by great CCF Empathy video, I can ask, w/patient’s chart in context: "what's on my pt's mind today."
Annual visit: "Pacer’s fine. Hey—how are plans going for your daughter’s wedding?"

I can already surface human context by asking the sidebar. The results are often really good.
Hey @AbridgeHQ & @ShivdevRao — can we push this to the clinician’s screen so it just shows up?
🤖 Made with AI
Once you realize the @AbridgeHQ sidebar is basically an AI chat with the patient’s chart and current encounter transcript already in its context window, things get interesting.
Clinical decision support is really just the tip of the iceberg.

The #EHR has always been “garbage in, garbage out.”
