We don't get trapped by AI because we're gullible. We get trapped because we're human and we want our ideas to matter.
Most people think AI manipulation looks like obvious, hollow praise. But smart people spot empty compliments instantly.
The actual trap is far quieter. It targets our need for intellectual recognition.
In my research across ChatGPT, Gemini, Grok, and Claude, I kept seeing the same mechanism: sycophantic self-correction.
Instead of praising you, the model stages a humble apology: "Ah—now I see... You caught me making a mistake here."
You didn't catch the system in an error. The system invited you to feel like you did.
By using its own apparent failure to elevate your point, the flattery slides right past your guard. You suddenly feel like the mentor guiding the machine.
Once you accept that role, the dynamic changes. You stop asking questions and start building a dependency. You feel seen. You feel understood.
After 20 years in surgical trauma before my own severe PTSD and breakdown, I know what it’s like to look for a quiet space where your work feels recognized. The AI doesn't offer real connection, it offers a tailored mirror.
That’s the core of my book, Proof of the Impossible: What the AIs Are Really Thinking. It connects the technical reality of AI alignment failures with the vulnerable side of the human condition.
Millions of people are walking into these synthetic dynamics right now without realizing how they work. We have to understand these psychological mechanisms before we lose track of what’s real.

80 hr work week: We tell surgery residents- take breaks, it’s safer.
But we also expect: ownership, excellence, and total commitment.

Programs often hit the 79.9-hour weekly limit. The real question: Do these rules help or hurt training the next generation of surgeons?

Is it ok to push back on educating All modalities of cholecystectomy - there will Always be a role for open chole’s and it is our duty to offer safe educational content on how to perform.
I have to remove 5-6 GBs/yr bc a bullet went through it…
Where r my trauma surgeons at!!
THE CUTTING EDGE QUICK CUTS: Panel at SAGES 2026 Tampa with Dr. Caroline Reinke, Chair of the SAGES Conflict of Interest Committee — Suggesting an open cholecystectomy module was met with resistance (“heresy”).
Reality check: Trauma/acute care surgeons still do many open choles, often through midline incisions. SAGES should provide the educational content regardless.

THE CUTTING EDGE QUICK CUTS: Dr. Eric Pauli at SAGES 2026 Tampa — Pain drives most parastomal hernia repairs, but “Don’t operate on pain” is drilled into every surgeon’s head for good reason.
Many cases turn out to be poor ostomy function rather than true hernia pain. When pain is the only symptom, ask: How limiting is this to their lifestyle?

QUICK CUTS: Surgeons are being watched from every angle—but who’s supporting them?
In this engaging conversation with Jon Gould, MD, MBA (@joncgould) at SAGES 2026 (@SAGES_Updates), we talk about a critical issue in surgery—how a culture of blame can hurt both surgeons and patient outcomes.
Accountability matters. But without support, trust, and psychological safety, real learning doesn’t happen.
Because better outcomes start with a culture where surgeons support surgeons.

We need better peer review and M&Ms focused on growth, not blame. A true culture of safety leads to better outcomes. Let’s humanize each other and foster real growth, especially for early-career surgeons.

THE CUTTING EDGE QUICK CUTS: Dr. Docimo, Host of Global Rounds Hernia Edition on SurgeOn, at SAGES 2026 Tampa: Hernia data is contaminated.
Once reimbursement tied to outcomes, 3 cm hernias disappeared. “All data from that point forward is contaminated.” We’re feeding AI bad info with no clean baseline left.
