Scotland put the 4AT into every orthopaedic unit. Screening went from near-zero to 87% in 4 years: 18,040 patients, no extra staff. The barrier is not time. It is the decision to start. #delirium #4AT

🧠 ⚠️ Delirium signifying acute brain injury = a massive risk factor for future dementia, *especially* in people with low baseline long-term conditions/frailty. This is not just an interesting epidemiological finding. ⭐ It is a major opportunity for dementia care - if acute brain injury during acute illness causes dementia we can work to understand how and then to design interventions. 👀 Dementia world - WAKE UP to delirium! #delirium #dementia Paper led by @rosespenfold - link in comments.

🧠 Calling all clinicians and researchers doing work on delirium! The deadline for submitting symposia for the upcoming meeting of the European Delirium Association is July 1st. The meeting is taking place on 4-6 November, 2026, in Oslo, Norway. This is one of the world's major international conferences on delirium. A wonderful place to present new data and ideas on delirium. Great CPD too! Link to the conference site is in the next post. #delirium

Remembering Dr. James “Jim” Rudolph, our founding President. 🤍 Today at #ADS26DEN, @smith_pedsmd shared something Jim told her in his final weeks: “At ADS we bring people together from near and far, provide a platform, and offer young people room to grow.” That was Jim.

🚨 The DSM is getting a major overhaul A new APA subcommittee just proposed a radical 4-domain diagnostic model that finally integrates biomarkers, transdiagnostic features, and social determinants into our standard nosology. In short: categories aren't enough. Thread 🧵

This is a map of repetition among all symptoms in the DSM-5. The majority of symptoms are found across disorders, yet we treat them as separate entities.

It's an excellent and pithy article. Strange, though, to see it under the "What's New in Intensive Care" section. The struggle to forge (and, crucially, get people to use) a unified terminology goes back a good many years. doi.org/10.1007/s00134-025-0… @KatarzynaKotfis

When to order neuroimaging in first episode psychosis - for everyone or just if we have suspicions of an ‘organic’ cause? We looked at MRI scan reports in people with first episode psychosis (n=1,486) using Clinical Record Interactive Search (CRIS) to find out
