~13yrs of neoadjuvant adventure comes to positive conclusion (for now)! In @NEJM today & presented at #ASCO25 5 yr survival for pts w lung cancer that can be removed by surgery is better with just 3 doses of immunotherapy before the operation! nejm.org/doi/full/10.1056/NE… #LCSM 1/

When we presented data from all the neoadjuvant/periop IO studies people were understandably concerned about what happened to those who didn’t get operated. People were also confused by the stage overlap between PACIFIC and the surgical trials. So the goal of MDT-BRIDGE was to better understand these gaps and evaluate whether an early MDT reassessment after 2 cycles could allow most patients to achieve definitive locoregional control, which they did in 95% of cases. MDT reassessment also enabled very high R0 rates. Complication profile for the CRT group seemed reasonable as well which was another gap in knowledge. The goal wasn’t AT ALL to compare the survival outcomes between the two forms of locoregional therapy. Our intent was to bring data to areas of uncertainty after multiple phase 3 trials. We also wished to understand why MDTs might interpret resectability differently and this work is still ongoing. The data are also not meant to be used to justify a trial. Before such an RCT could be done, I think the Rad Onc community would have to prove that neoadj chemoIO followed by CRT is superior or at least equivalent to PACIFIC. Once this is done, we can talk about the RCT… still much work to be done.

Remarkable results from SBRT +/- immunotherapy for unresected stage I-II NSCLC. No benefit from adding IO & surgery remains standard of care for fit pts w stage I-II, EFS of ~30m w SBRT vs 46-65m for much more advanced disease in trials of neoadj IO #lcsm ascopubs.org/doi/10.1200/JCO…

🔥 Outstanding piece of work published today in Nature Medicine! Huge congratulations to @MartinReck2 ,@NadalErnest , @DoctorJSpicer and @AndrearicFili , and the entire MDT-BRIDGE team on this important publication! 👏👏 The phase II MDT-BRIDGE trial, investigating neoadjuvant durvalumab plus chemotherapy in resectable or borderline resectable NSCLC, tackles a clinically relevant question: how can we push the boundaries of multidisciplinary treatment and offer more patients a chance of curative-intent therapy? This is exactly the kind of collaborative clinical research we need: challenging established boundaries, asking meaningful questions, and ultimately striving to improve outcomes for our patients. Congratulations to everyone involved! 👏 🔗 nature.com/articles/s41591-0… Happy to continue sharing exciting advances in lung cancer research as an #ESMOAmbassadors #ESMO26 #NatureMedicine #LungCancer #NSCLC #ThoracicOncology #Immunotherapy #ClinicalResearch #MultidisciplinaryCare #CancerResearch

Excellent discussion by Big Ben @benlevylungdoc Of thoracic onc - great points as to pts w tps>50 advanced NSCLC/ PARSEE trial ☑️ what we will do in clinic tomorrow ☑️key data coming to shape this space ☑️how this data will impact emerging IO+/- trial data #lcsm #ESMO26
PARSEE trial- while we might be parseeing the data for a while, on surface they seem per se immediately practice-changing! Chemo/IO led to impressive benefits for tps score>50 pts over pembro alone when it comes to primary endpoint of PFS, trends for OS, dramatic reduction in early progression (25 to 5% or so) - at cost of anticipated toxicities Benefit surprisingly just as much in tps >90! Only subset of note w limited benefits - pts>70 yrs So it seems chemo/IO is now the best SOC for pts w non-AGA tps>50 advanced NSCLC w IO likely most appropriate for frail elderly Special kudos to Dr. Decroisette and the French GFPC study team for completing such an important trial! Merci! #lcsm #ESMO26
