This is a big deal in Investigational New Drug (IND) applications. It is now possible to reduce the time it takes to submit these from 3–4 months to a month; soon it’ll be down to days, greatly accelerating new drug submissions for clinical trials & saving tens of millions of $!
Writing an Investigational New Drug (IND) application is a major hurdle for drug developers. It can take a team of specialists three to four months, and mistakes can trigger protocol amendments or clinical holds. In June, we started working with Population Health Partners (PHP) to leverage specialized Kosmos Clinical Protocol Design (“CPD”) and Kosmos Regulatory Authoring (“RA”) products to support PHP’s process of IND preparation. Since then, specially trained Kosmos products produced a first acceptable draft of the Nonclinical Overview in 4.2 hours, against 100 hours of expert time, flagged an error that roughly fifteen human reviewers missed, and authored 70% of the text in a Pre-IND Briefing Meeting Request submitted to FDA. PHP is now bringing Kosmos products to a second program and expects to complete IND preparation in as little as 1 month, instead of the 4 month industry standard. PHP's experts can spend their time reviewing and deciding, not just drafting. The time recovered by Kosmos products means that drugs could make their way to patients faster.

I’m no economist, I have a basic knowledge at best. Maybe it’s just bad news bias, but the UK outlook feels bleak to me. - The FTSE 100 is at a 3-month low - Inflation is up to 3.1%, 4% expected early next year. - Energy costs are surging with diesel heading for £2 a litre - 10-year gilt yields are around 5.45%, the highest since 2007. On top of that, more high earners are reportedly leaving. If this keeps going, are we heading for a recession, or am I blowing the figures out of proportion?

Data about how referrals are made still feels like the most underutilized dataset in healthcare. Being able to understand the referral patterns from a certain doctor or to a certain doctor/hospital can help inform a lot about how the healthcare system gets used, understand why certain choices are made, and potentially intervene if the referral is a wrong fit. But referral data is largely dark matter that exists in a liminal space between the two providers. It’s inferred from other datasets like claims or EHR notes, but it misses a lot of the nuance that’s actually sent in the referral transmission. But if this data can be captured and structured easily now, I think it can be a really valuable dataset. Here is an interesting study. Within a health system, when primary care makes a referral to a specialist only 35% had a documented completed appointment. But looking at the methods, every out-of-network referral automatically shows as "no status" (~10% of referrals) because Duke's Epic can't see what happens at a non-Duke specialist. Another massive chunk (~20%) of tickets had no date because schedulers booked appointments without linking them back to the original order. Just goes to show how hard it is to do analyses with our current available data, especially across providers.

A student loan now takes 9% of everything earned over £25,000, for forty years. It comes off before the money lands, on top of income tax and National Insurance, and the balance grows with interest the whole time. Most people will never clear it before it gets written off at the end. Forty years is longer than the mortgage most of them won't be able to get.

Wages are the most heavily taxed way to receive money in this country. Income tax up to 45%, with National Insurance on top of it. Money that arrives from owning something instead — dividends, or an asset sold at a gain — is taxed at noticeably lower rates. The system is built to reward owning. Almost everybody is set up only to earn.

Two fascinating cancer breakthroughs are happening at same time. First, a engineered probiotic bacteria attack 43% of colorectal tumors in mice. This bacteria can produce chemotherapy inside tumors while also activating the immune system. In mice, this treatment completely eliminated 3 of 7 tumors — ~43%. Then, in another study, a drug wiped out all detectable disease markers in 78% of high-risk patients before they developed full-blown multiple myeloma. Researchers tested teclistamab in people with high-risk smoldering multiple myeloma, a precursor to active multiple myeloma. As a result, 77.8% of patients achieved a complete response or better. At 24.5 months, 92% remained progression-free. That indicates cancer treatment is getting more programmable day by day.

Sales leader friend at one of the payer AI companies sold triple his quota in 6 months. The payer momentum on coding review and denial solutions to keep up with the provider AI tooling is unbelievable. But I think the more interesting thought is to predict what happens in a few years when provider and payer AI is perfectly coding and utilization managing every claim --> I expect more payers to go into APMs or direct contracting with providers as one of their last remaining options in managing expense.
