A major step forward for women at #ESCCongress 🫀
The new 5th Universal Definition of MI explicitly requires sex-specific 99th-percentile troponin thresholds recognizing that using the same cutoff in women and men can systematically underdiagnose myocardial injury in women
It also recognizes SCAD, coronary vasospasm & embolism as causes of primary MI—mechanisms especially important in women.
Women’s heart attacks don’t always look like men’s. Our definitions finally reflect that. We wanted to enforce this in the Cheat Pain guidelines but we didn’t have enough data in the US at that time. Glad this has changed!

#DOMINO registry on MINOCA presented by @DoryMD #ESCCongress with senior author @anagalmeida1 ➡️ prospective, international registry of 365 MINOCA patients across 17 centres in 13 countries (2023–2025), designed to measure how often @escardio guideline-recommended advanced testing (CMR, intracoronary imaging, invasive functional assessment) is actually used in real-world practice, and whether it’s linked to reaching a specific etiologic diagnosis. Congratulations to Doralise and the entire group!
🔥 5 key points 🔥
1.Testing was underused, 30.4% got no advanced testing, 34.2% no CMR, 95.1% no functional assessment, with use ranging 0–100% across countries.
2.Testing was linked to reaching a diagnosis, OR ~3.7–3.9, significant in two of three models, with resolution rising stepwise from 85.6% to 97.1%.
3.The link is partly definitional, since CMR and functional testing are themselves used to assign diagnoses, so part of the benefit is circular.
4.The sex gap in CMR is explained by age, women were ~14 years older, and the disparity vanished after age adjustment.
5.A treatment gap persists after diagnosis, e.g. calcium channel blockers newly prescribed in only 25% of eligible spasm/microvascular patients.

This is the first statement to define what a Women’s Heart Centre actually is: its structure, its team, its referral pathways, and the training standards clinicians need to lead one.
The problem: CVD is the #1 killer of women. Yet women face systematic delays in diagnosis and treatment and the conditions most likely to affect them (INOCA, MINOCA, pregnancy-related CVD) remain underdiagnosed everywhere.
Our solution: hub-and-spoke Women’s Heart Centres, embedded in existing CV systems, not parallel structures. Advanced diagnostics, multidisciplinary care, research coordination.
For the first time we define:
✅ The nucleus clinical team
✅ Adjunct specialties (cardio-obstetrics, oncology, rheumatology, psychology and more)
✅ Core vs advanced competencies
✅ A pathway toward subspecialty certification
We also set milestones for national and institutional adoption.

Integrating AI into daily life at the echo lab🫀
We were pleased to welcome Dr. @FedericoAsch , director of the Echocardiography Core Lab at MedStar Health Research Institute and Professor of Medicine at Georgetown University, to deliver the @ASE360 keynote lecture at #CSEcho2026.
Dr. Asch sparked insightful discussion on clinical applications for AI and its potential impact on workflow efficiency, reproducibility, and quality assurance in the echo lab. This lecture highlighted how AI-enabled tools can be thoughtfully integrated into echo lab workflows while maintaining full clinical oversight and interpretive responsibility.
What an important and timely conversation as innovation in cardiovascular AI continues to evolve!
