The swan vs. echo debate is such a false dichotomy, great article/post below 👇
Swan complements echo assessment. Now, if I only have 1 option, I would choose echo as you can get anatomy + some hemodynamic measurements, but for sick deteriorating shock patients, having both is valuable.
Much easier to try a hemodynamic intervention and repeat thermodilution on swan to get a new CI than it is to repeat a VTI on echo. So depends on how many sick patients you are managing at once!
Echo tells you the heart is struggling. The PAC tells you why: pump failure, vasoplegia, low preload, or a failing RV. Don’t fly blind. 🎩 tip to the authors.

Great case, difficult question:
Large RP bleed. Abdomen is rock hard. Ppeak on APV 60 (left pic). Diagnosis? Abd compartment sd.
Treatment? Decompressive laparotomy. See Ppeak at 41 on the right after decompression.
Something on the pics doesnt fit this scenario. What is it?

Thank you for posting this @accpchest! #VExUS is a heated topic at #CHEST2026, and I'm glad it is out! Huge thanks to @NephroP and @ArgaizR for their expertise with such an insightful discussion. Many thanks to Dr. Nick Villalobos and the CHEST Ultrasound and Chest Imaging section team for their organization!
Listen to this podcast for to hear perspectives on POCUS evaluation of venous congestion, including the venous excess ultrasound (VExUS) exam approach. Led by the CHEST Thoracic Oncology and Chest Procedures Network’s Ultrasound and Chest Imaging Section.

Many nephrologists who dismiss #POCUS with “scanning the heart isn’t our cup of tea” simply haven’t seen its value at the bedside or worked with colleagues who could demonstrate it.
Come hear from those of us who use it routinely in clinical practice!
