Coming to you live from the ED to tell you emergency medicine is really fucking hard.

Hi, medical toxicologist here. Administering large doses of naloxone to all patients with opioid toxicity to "totally reverse" them is not a good take. My thoughts below:
When it comes to naloxone opioid reversal, there are 2 schools of thought - give them just enough to increase their RR or totally reverse and wake them up. I'm in the latter. I rarely start with <4mg.
There's a theoretical risk that their opioid could outlast the naloxone, they wake up, refuse transport or leave the ED, and then drop dead. That's another myth. In a study of 542 patients completely reversed by EMS followed by refusal of transport, there were 0 deaths within 48 hours. So, the moral of the story is that either method is likely acceptable and safe.

Any articles on how preceptors can reflect/track/incorporate learner feedback? I'm finding zero and am starting to think it's not talked about enough. We're doing learners a disservice by not adapting and improving. Any articles on this out there? #twitterx #academia #residency

🔍The search is on for the #EMPRx24 Pharmacy Awards. Who's setting the standard in your ER?
✨ Nominate an EM PharmD who’s gone above and beyond. Know a non-pharmacist who’s a pharmacotherapy pro? We want to hear about them too!

Stewardship initiatives that would work, would not cause harm, but no one would let me do:
1. All providers get a daily vanco ration. If you run out, you have to borrow from other provider, they have to agree w/ you and give you one of theirs
2. Board w/ # quinolone Rx by name

Going into the holiday, a few reminders:
Colloquially it’s called Independence Day.
To ER doctors it’s ‘omg don’t drive drunk you’ll all just hit each other, seriously don’t hold fireworks with your hands, and that potato salad been sitting out for 9 hours now, don’t do it’ day
Officially PGY-DONE. So grateful for the opportunity to have worked with and learned from @IUSMEmergMed. I couldn’t have imagined a better year.
I’m excited to be starting my career as an Emergency Medicine Pharmacist at @uchealth!
