Israel. Reported rates (UNODC/World Population Review, recent years) put it at ~15.5 per 100k. Next are Turkey (~15.4) and Morocco (~2.7); most others (Egypt, Saudi, Lebanon, Palestine, UAE etc.) are under 1.
