This a million times. I’ve seen far worse end stage OA in physically inactive metabolically unhealthy patients than marathon runners or ex athletes.
We don’t see too many patients who’ve “worn out” their knee or hip through regular exercise. Osteoarthritis seems much more of a metabolic disease. Try to wear your knee out with physical activity (you won’t); your heart will thank you! And please, don’t say wear and tear!! 🤦

Consent ✅ Guess what Male, 50s - Marathon training, increased mileage & speed work L groin & thigh pain, limping, night pain Treated as hip flexor tightness by physio +ive Hop test, +ive FADIR test Weakness & pain on iliopsoas testing MRI - high grade medial femoral neck of femur stress fracture - involving the iliopsoas tendon insertion at the lesser trochanter Diagnose these as the hip flexor at your peril ENDURANCE RUNNERS WITH A RAMP IN TRAINING DON'T GET HIP FLEXOR PROBLEMS THEY GET STRESS FRACTURES 😡😡😡

Many think osteoarthritis (OA) is a 'wear and tear' problem. While the instigator may be traumatic (think ACL injury), framing it as wear-and-tear can cause patients to think exercise will make things worse. So understanding why cartilage loves load is important...🧵👇 1/7

Fascinating paper. It seems not all plaques are created as equal
Regular exercise reduces the likelihood of plaque accumulation in the arteries However, many long-term exercisers appear to have coronary artery calcification, indicating atherosclerosis progression The fascinating thing is that despite the higher plaque, those people still have lower rates of cardiovascular disease events and mortality Full video youtu.be/S-NKPZSkbXg Graph: academic.oup.com/eurheartj/a…

Important infographic on management of OA. Focus on exercise and weight management as front line treatment. Pharmacology has moved away from opiate based meds to short term nsaid use. #msk @NHS_LLR @llrpcl

Brilliant teaching session from Mr Basu this evening from @Leic_hospital for me on spinal pathologies for GPs. A lot to take away from the session to improve patient care as well as fostering good cross-relations from primary and secondary care @llrpcl spinesurgery #msk
