Main messages that have resonated to me at #AIBD2024 as a Crohn’s patient;
⚫️ focus is often too much on risk of therapies and not risk of IBD
⚫️there’s no place for step up therapy
⚫️sometimes surgery is the safest treatment
⚫️Mediterranean-style + plant focused diet best

-Pts with CD on combo therapy + steroid free remission > 6 mo ~similar relapse rates between IMM withdrawal versus continuing combo therapy (10% vs 14%).
-A/w higher risk complications s/p endo dilation = multiple strictures in short segment, deep ulcers, stricture > 4-5 cm

Ex: macular edema
reversible but risk higher in DM. Consider relative contraindications based on patient hx.
-CRP & WBC lvl can be predictive of relapse risk at Year 1 & Year 2 of weaning off IMM in combination therapy.
Good reviews and discussion this morning:
-TNFi dose reduction more successful if no prior TNFi failure + deep remission.
-S1P receptor mods AEs = guided based on patient-specific factors, positioning after 5-ASAs.
Great presentation/discussion this morning on nutrition & medical + medical management in J-pouch pouch. ?timing of surgery, fiber/hydration misconceptions, avoiding “normalizing” symptoms. @shannonchangmd @MaiaKayalMD Stacy Collins #CCCongress24

❗Huge win for #priorauth reform❗ A new rule issued by @CMSGov will require payors to respond to urgent requests within 72 hours for an expedited request or 7 days for non-urgent appeals. Get the details ➡️ ow.ly/TSK550Qs76M

🌎 to @BCBSMA – patients expect anesthesia for a colonoscopy! Why add a barrier to #coloncancer screening? Stop policy 154 before it goes into effect Jan 1.

Amazing talk by @horstibddoc on a highly prevalent/impt topic: #Fatigue in #IBD
How to approach ✔️
1. Treat active #IBD
2. Rule out side-effects

The road less traveled…
It’s been like clockwork - sitting for a “big” certification exam every 2-3 years: Internal Medicine, Psychiatry, and now GI. Hopefully the last one for a while!
Thank you to my past & current mentors @uihealthcare @ClevelandClinic, currently @Intermountain. Drs Judith Dayan, @JPAchkarMD @MRegueiroMD @IBD_FloMD
Special thanks to @DDrossman for guiding and showing me that the road less traveled bears its own unique perspectives in patient care and @NateMerrimanMD in being forward thinking as we build IBD care @Intermountain !!
