I’m doing some book keeping as a diversion for the next time someone tries to talk to me about basic science. An #anaesthesia 🧵 of 🧵’s regarding some of the things I think about during my day job. (Thank you to everyone that has contributed to the discussion) 1: Alfentanil
I don’t normally geek out about #anaesthesia but given the ongoing Remifentanil shortage in 🇬🇧 I thought I’d post my alfentanil TIVA recipe. IMHO it’s elegant, precise and allows for absolutely minimal anaesthetic poly pharmacy. (Very boring for non anaesthetists…..) 1….

ITU have clearly lost their way Allowing people without a medical degree to look after the sickest patients in hospital is a total nonsense I’m just happy we’re at the naming & shaming trusts publicly stage I will be avoiding @UHSFT 🫡
Every medical/ICU consultant enabling this practice should be publicly named and shamed! Clearly ‘noctors’ rarely care about patient safety and outcomes, and corporate managers care only about money, but how many more people have to die before we see significant change? Terrible!

Doctor substitution is being championed at the highest levels yet anyone with a grain of common sense or integrity knows it’s nonsense I wonder what platitudes @theRCN or @RCEMpresident would offer these parents given their involvement in enabling this scenario?

GP refers child to hospital appropriately Nurse doing a job they should never be doing, ignores referral, misses the diagnosis a third year medical student would make & inappropriately discharges child to worse possible outcome Dropping professional standards causes harm : FACT

The anonymous contributor list to the latest PA document is so entirely on brand for a project mired in conflicts & opaque “evidence” since day 1 How can anyone in healthcare leadership bang the transparency drum whilst thinking this is ok? As appalling as it is unsurprising

Anyone that defends this level of welfare spending is the problem No nuance, no ifs & buts, this has become an exercise in shameless taking and there will soon be an exodus of very mobile, high rate tax payers that have had enough Then what…… ?

No-one reads my football tweets but it’s clear to anyone with a 6 year old’s numeracy City have been cooking the books for donkeys The fact they’ve been caught red handed whilst ownership remain obnoxiously defiant is a case study in our post accountability era Bury them 💀
BREAKING: Manchester City statement. ⤵️ Manchester City FC is both disappointed and surprised by the opinion of the Premier League Commission, that has been published today. The Club is innocent of the accusations made by the Premier League and a comprehensive body of irrefutable evidence exists in support of all of its positions, relating to this case. The Club will therefore be relentless, and where necessary proactive, in any and all appropriate regulatory and legal forums.  The Premier League process remains ongoing, with significant elements uncompleted. Manchester City FC will now pursue the appeal avenues open to it, on the basis that the opinion contains clear material errors, of law, principle and fact, and is unsafe.  The Club has diligently respected due process for eight years on the basis that the Premier League Board and Executive would behave as an independent, impartial and fair-minded regulator, free from partisan influence.   The Club is obviously restricted in what it can say further until all future proceedings are complete. mancity.com/news/club/manche…

Today I learnt that saying only people with a medical degree should work on doctors’ rotas is an extreme, polarising view 👀 Which is weird because I always thought that should be blatant common sense… …or else what’s the point of medschool? 🤷‍♂️

If someone doesn’t understand the difference between evidence of absence & absence of evidence…… I have very little interest in anything else they have to say It’s evolved into an excellent screening question

If you substitute those with the longest & most rigorous training in healthcare it will result in harm There is literally no other logical outcome and it will soon be evidentially undeniable too I wonder if there will be any accountability ? 🤷‍♂️

Two things can be true at the same time Welfare payments are as necessary as they are also totally out of control atm Unless someone addresses this the UK is going both economically & socially bankrupt We’re about to see the Overton window shift very quickly on this!

If I’d sent this about nursing I’d expect widespread rebuke & a meeting without ☕️ Dean QN, who I thought was sensible, has the hump because he’s been saying silly things & subsequently winning silly prizes As a side note, I’m always partial to peoples’ tweets after midnight 🌝

So a 20 year old can be in the “second highest ranking position in local authority” without any relevant qualifications or life experience whilst weirdly bragging he couldn’t even get hired to flip burgers? Tax payers deserve so much better
'I handle a £100 million budget for a London council, but I can't get a job at McDonald's.' Deputy Mayor of Hackney Dylan Law explains how tough the job market is for young workers, and he thinks he's living proof.

Negligence claims rising over roughly the same time period doctor substitution has massively increased, especially in ED’s Clearly this isn’t evidence but it’s logical to suggest that neglecting appropriate qualifications causes harm Any journalists want to join the dots?

Diagnosing new undifferentiated patients is the nucleus of medical education It requires completion of a highly competitive & academic course followed by years of rigorously examined postgraduate training It’s staggering the false equivalence claims some are trying to peddle
Well we agree on that. There is a difference in the roles and the training. Seeing new undifferentiated patients requires different competencies which is why ANPs train for longer at MSc and with mentorship and supervision in the areas they will practice in.

During Covid my colleagues & I had our lives upended not only having to watch all ages die but also instantly change our schedules to 12hr day, 12 hr night, day off on repeat for months. Many of us with young families. A lot of people need to grow up or HCP’s need more respect!
Some words on my return to Parliament this week, including a huge thanks to my constituents for their understanding & compassion while I've been working on my recovery from burnout. I've been blown away by your kindness 🫶 Looking forward to getting back into things. Onwards 💚

There’s only one reason why any Brit would go to medical school in Bulgaria and it certainly isn’t superior equipment in their hospitals 🙄
Replying to @MerrynSW
I have two clients who went to med school in Bulgaria because the equipment in their university hospitals was far superior to what we have in the NHS. There’s also a cap on the number of medical school places in Britain. Interestingly, they’re both privately educated, very smart kids from HNW families, and chose to study abroad instead of here. I searched to see whether there was any data supporting a broader point, and 70% of doctors joining the UK workforce with Bulgarian medical degrees in 2023 were actually British nationals. That feels like a pretty obvious failure of UK medical training capacity.

Curious about the further education sector now that everyone’s finally waking up to the fact a pointless degree from a second rate University does zero for job prospects but does leave you with £50k of extremely high interest debt Going to be a lot less Unis in 10 years!
