Name a more iconic pharmacist phrase. Also I think I need to step it up and stop free-handing these projects 😬 #TwitteRx

My pronouns are she/her. EM is despicable and you can’t convince me otherwise. Thankful for the time I spent here, the people I’ve met, things I’ve learned but it’s truly not worth it for me anymore. Ttyl #TxPharm #TwitteRx see ya on another platform

Seeking clarification/advice from those who have done this before: Got my board certification (BCTXP) in spring of 2022. Received an email that says recert period begins 2023. Received another email saying I’m behind because I did not log CE for 2022. WHICH IS IT? #TwitteRx

For those wondering, the denial was for isavuconazole due to failure to try and fail terbinafine…for pulmonary aspergillosis…
Best way to describe a recent PA denial: “You request almond milk for a recipe that calls for whole milk because you’re lactose intolerant. You are instead offered an egg, because one egg is far cheaper than a carton of almond milk, even though it doesn’t do the same thing.”

Trying to see something…your patient is intubated and has a functioning feeding tube. Which formulation of tacrolimus do you go with? #TxPharm
21%Sublingual
79%Suspension via tube

PSA you can in fact vote today if registered as independent. These aren’t primaries. PA is electing a brand new governor and senator as a result voters today. This may seem obvious but this misunderstanding just came up with a loved one, not sure how widespread it may be.
That’s my RPD/colleague! 🥳🤩

In the perfect example of how life rarely goes to plan, we moved back to Philly, got the dream job close to all our family, and yes that turnaround was fast. Excited to join the lung txp team at Penn this week! 🫁♻️ Brink is enjoying being a Philly dog so far ☺️

It was a joy having Hanna on rotation and seeing how much she learned and grew in just a few short weeks! Rounding out the month with a quick look at bela-based immunosuppression. #TxPharm #TwitteRx
Final #TakeoverTuesday with Hanna! “Belatacept is a selective T-cell co-stimulation blocker that is FDA approved for maintenance immunosuppression in kidney transplant patients in combination with mycophenolate and steroids, as part of a CNI-sparing regimen. (1/x)

Finally achieved a major life goal: adopted a dog! I’m obsessed already. Also fun vet pharm tip: this little guy was treated for heart worm, and he’s now on prednisone. We put the pill in his food, drizzle a 💧of honey on it, and boom, med taken!

Paxlovid only available at certain pharmacies is actually a HUGE med safety issue. Don’t tell me America has the best healthcare system until we have reliable medication lists at all phases/places of care. DDIs are easily missed in the current setup. #TxPharm

Happy holidays #medtwitter. Thank you for your work. Always remember the call to the family contact w/ updates on your patient on ATC opioids and only 1 half-usable arm means the world, esp w/ strict visitation restrictions. One day we may laugh about this but certainly not today

Do we fault the institution or the journal? Not every clinician needing these articles has all the $$$. Also, how much do the investigators have to pay to submit their work to such a journal? What a system we have.
When you find an article that might have the answer to your question but your institution doesn’t have access to it.

“Can we dose this BID instead of QID for an easier regimen?” “I’m going to retime the Lasix so they can sleep tonight” “I’ll talk to her about switching immunos, I know she has a lot of ?’s about it” This motto hangs in my office. No intervention is too small.

I always knew I wanted to expand clinical pharmacy services into outpatient settings, but it is HARD to get up and running. Shout out to all the trailblazers out there trying to bring patient care to a new level across the spectrum of care!
