Honored and deeply grateful to be recognized by @SCAI with the 30 in Their 30s award. This recognition reflects the mentors who guided me, the colleagues and friends who supported me, the fellows and trainees who continue to inspire me, and the patients who place their trust in us every day. This year has been especially meaningful with several personal and professional milestones that made me reflect even more on how much of this journey is shared. I am thankful to everyone who has been part of it and excited for what lies ahead. Congratulations to all of the other recipients. It is an honor to be recognized alongside such an inspiring group. @SCAI_Prez @RowanVirtuaSOM @VirtuaHealth @TCTMD @crfheart @SVM_tweets @JeffHealthEH @ACCmediacenter @American_Heart @ACCinTouch

His legacy will live on. May his soul rest in peace 🙏🏼
The American Heart Association mourns the passing of the legendary cardiologist Eugene Braunwald, M.D., widely recognized as one of the most influential figures in the history of cardiovascular medicine. Over seven decades, his work reshaped the understanding and treatment of heart disease, leading many to call him the father of modern cardiology. Braunwald was a lifelong contributor to the American Heart Association, helping advance its research and scientific mission, and was honored with some of the Association’s highest honors for his lasting influence on cardiovascular care and research. His influence extended well beyond his own discoveries, as generations of Association‑supported investigators, clinicians and academic leaders were trained by Braunwald or guided by the clinical trial standards and mentorship models he helped establish. newsroom.heart.org/news/amer…

How the data emerging from #ACC26 will shape my thinking No single trial should redefine practice. Decisions remain anchored in the totality of evidence and the realities we face at the bedside. Axial mechanical support trials including PROTECT IV trial and CHIP-BCIS3 trial reinforce that high risk PCI is not a uniform entity. The signal is not universal. The opportunity lies in identifying the specific hemodynamic and anatomic subsets most likely to benefit from upfront support. LAAC data remind us that stroke risk in atrial fibrillation is not eliminated with closure. Contemporary anticoagulants carry a low risk of non major bleeding, a threshold that may fall further with emerging factor XI agents. Outcomes with WATCHMAN FLX appear favorable in selected lower risk patients, but potentially at the cost of higher stroke rates. Caution remains warranted before committing low bleeding risk patients to a permanent implant. IVUS in PCI continues to show incremental benefit, yet outcomes in high risk PCI appear similar with or without imaging. This is not unexpected. Operators who routinely use IVUS tend to internalize principles of sizing and optimization. The more relevant comparison may be between low and high IVUS utilization, and between early career operators with robust imaging training and those without. Catheter directed thrombolysis using EKOS system remains a sound option for intermediate risk pulmonary embolism when bleeding risk is acceptable, as supported by HI-PEITHO trial. While recent momentum has favored mechanical thrombectomy, it is reassuring to see continued support for CDT. In patients undergoing TAVI, routine PCI of stable coronary disease is not supported. A selective approach focused on lesions with clear prognostic relevance is more appropriate, though the field has yet to precisely define this threshold. AngioFFR is emerging as a practical method for ischemia assessment. Wire based physiology remains reliable and essential in specific settings. However, unlike wire based tools, angiography derived algorithms will continue to evolve with iterative learning, much like CT-FFR, and are likely to become increasingly central in routine practice. ORBITA-CTO trial adds an important perspective. CTO PCI offers meaningful improvement in symptoms and quality of life in carefully selected patients. There is a clear signal of benefit. For patients with refractory angina despite optimal therapy, CTO PCI should be viewed as a legitimate escalation strategy. @ACCinTouch @SCAI @TCTMD @RowanVirtuaSOM @SVM_tweets @TCTConference @TCT_ME_ @American_Heart @AHANewJersey @AHAPennsylvania @mmamas1973 @mirvatalasnag @djc795 @MichaelMegalyMD @GPAngioClub

In patients with symptomatic single-vessel chronic total occlusion, percutaneous coronary intervention for coronary chronic total occlusion (CTO PCI) improves angina beyond placebo. Get the details in #JACC: jacc.org/doi/10.1016/j.jacc.… #ACC26 #PCI #CTO @dr_skhan1

Well those #IC trials at #ACC26 really delivered! Even go the nonIC people, we see these as game changers! 💥From CTO symptom relief (ORBITA-CTO) to evolving physiology guidance and LV support in high-risk PCI—these trials refine how we treat patients Practice may not shift overnight—but thinking should

Excellent data to support CTO PCI for symptom relief & quality of life improvement in select patients. This debunks the “sham effect” CTO PCI is often labeled with as it was directly compared to a sham procedure. Medical therapy is often inadequate & intolerable. Bottom line for those patients offer CTO PCI #ACC26 #CHIP #CTOPCI #SCAI @SCAI @ACCmediacenter @American_Heart @TCTMD @JACCJournals @ACCinTouch
1/16 🧵 ORBITA-CTO: The first randomized, placebo-controlled trial of CTO PCI. 📰 Published in JACC, presented at #ACC26. 🔬 Finally, sham-controlled evidence for what we do in the cath lab every day. One of the best designed studies I have ever seen! A biostatistical deep dive. Let's go. 👇

#ACC26 What an incredibly challenging RCT. ORBITA CTO. Simple question with amazing blinding and patient reported angina frequency based on app (much better than recall). CTO PCI DOES improve angina much more than meds in JCTO 3 or less.

Cardiology has spent years talking about resilience, grit, and performance. We have spent far less time confronting the cost of a culture that too often normalizes silent suffering. A recent JACC piece and its companion global survey should force that conversation into the open. More than 1 in 4 cardiologists reported a self-identified mental health condition, and the burden was not evenly distributed. Suicidal ideation was present. Women reported considering suicide more often than men. Hostile work environments, discrimination, and emotional harassment were strongly linked to worse mental health. These are not peripheral wellness issues. They are signals of a profession that still too often asks people to endure rather than heal. If we are serious about protecting the future of cardiology, we need to stop treating physician suicide and mental health as private failures of coping. They are also structural failures of culture, leadership, and psychological safety. Confidential access to care should be easy. Seeking help should never feel professionally dangerous. Harassment and discrimination cannot be tolerated as background noise. And leaders in our field should say plainly that being accomplished does not make us immune to distress. We are never immune to influence, exhaustion, shame, or isolation. Advocacy in this space means building a profession where physicians do not have to choose between appearing strong and staying alive.

People already ask AI for medical opinions. The general public should consult with their physician before implementing any treatment. Use the AI generated content to inform the discussion with the physician. Uploading medical data, even if de-identified is a privacy and security concern. You have no idea who can access/use that information. Once it’s out there, it’s out there. @elonmusk
You can just take a picture of your medical data or upload the file to get a second opinion from Grok

Important points from Day-2 Discussion #CTO2026 Before you escalate the wire, watch what it is already telling you. • If the wire starts freely “flopping” and you see PVCs, you’re likely in the ventricle. Get out of that branch early. • Repeated wire entry into the same branch usually means your trajectory is wrong. Change your angle. • Stable catheter position + controlled contrast injection dramatically improves visualization. • Keep the catheter high and steady, let the contrast column guide you rather than chasing shadows. @crfheart #CardioX #CardioTwitter #CardioResearch #CardioEd @esbrilakis @colletcarlos @Ganeev @BinitaShahMD @DrBillLombardi @mbmcentegart @JWMoses @wjn_md @jgranadacrf @MartyBLeon @triciarawh @TCTConference @TCTMD @jedicath @Babar_Basir @MinaFar20 @MichaelMegalyMD @ShariqShamimMD @Shoaib9702 @Tabaza @ladyheart_md
