The “more than doubles prostate cancer risk” line is being treated like settled law. It isn’t. That number comes mainly from one 2009 trial (AFPPS) in which men took 1 mg of folic acid a day as a pill; four times the amount the UK is adding to flour (250 µg per 100 g of non-wholemeal wheat flour). In that trial the 10-year prostate-cancer probability was 9.7% vs 3.3%. A 2013 individual-patient meta-analysis of ~50,000 people in randomised trials then found no statistically significant rise in prostate, colorectal, lung or breast cancer, and said the AFPPS signal was most likely chance. EFSA’s 2023 review reached the same conclusion: evidence is insufficient to claim a causal link at fortification doses. Two different things are being smashed together: high-dose supplements and low-dose flour fortification. Natural food folate does not show the same signal. A 2024 Mendelian randomisation paper found a modest association with supplementation (OR 1.20), not with dietary folate equivalents. That is not “laws that make people sick.” It is a contested, dose-dependent finding being used as a slogan. The policy itself is narrower than the panic. Only non-wholemeal wheat flour is covered. Wholemeal is exempt. Organic and speciality flours still exist. The stated aim is to prevent about 200 neural-tube-defect pregnancies a year because nearly half of UK pregnancies are unplanned and the neural tube closes by week four, before many women know they are pregnant. The US and Canada have done this since the late 1990s. You can argue the trade-off, the lack of choice, or MTHFR metabolism. You cannot honestly say the science shows fortification “more than doubles” men’s cancer risk. That claim is the high-dose pill result, stripped of dose, duration and later evidence.
