🚹The results of the 2025 @AmerMedicalAssn prior authorization physician survey have been released! And they reveal that PA is STILL a significant burden for patients, providers, and the health care system as a whole. ama-assn.org/system/files/pr

NEW: The Trump admin is using AI to analyze Medicare prior authorizations, and companies get 25% of the "savings" the system generates. We found Carlyle Group and Patient Square Capital, private equity firms, backing this process. It's the latest oligarch attack on healthcare.

Oh my. The Secretary of Health and Human Services is investigating “chemtrails.” So apparently it’s time for some middle-school science: Jets fly high. It’s REALLY cold up there. Hot, moist exhaust hits cold air. The water condenses and freezes into tiny ice crystals. That white trail behind the airplane? It’s called a contrail. We’ve known how this works for a very long time.
RFK Jr: "We are also looking at chemtrails"
Readers added context they thought people might want to know
RFK Jr. referred to "chemtrails," a conspiracy theory alleging secret chemical spraying by aircraft. No evidence supports this; trails are condensation (contrails) and a 2016 peer-reviewed survey of 77 atmospheric experts found 76 had seen none for such programs. epa.gov/regulations-em

The AMA has filed suit: “Notwithstanding the lack of legal authorization, Wisconsin optometrists have attempted, and continue to attempt, laser and other surgeries on patients with impunity, causing adverse outcomes for patients,” the complaint says. 👓 tinyurl.com/2wp68tkf

I win again! Newly-released Swedish data shows that the rise in autism between 2001 and 2020 was... ENTIRELY attributable to an increase in cases WITHOUT intellectual disability—less severe cases than used to be the norm! The autism epidemic is not real: we just diagnose more.

BREAKING: The US Government officially posts its largest July budget deficit in history, at -$432 billion, due to an acceleration in federal spending. Interest on US debt rose +$26 billion from last July's levels to an alarming $118 billion for the month. This puts total interest expense for FY2026 up to $1.17 trillion in FY2026. As a result, interest expense has officially surpassed both National Defense and Medicare spending. In other words, the US government now spends more money just on interest than it does to fund the entire US Military or to provide healthcare for seniors. We cannot afford higher interest rates.

Vaccines are among the most thoroughly studied, effective tools we have to protect children from serious vaccine-preventable diseases. Decisions about how and when vaccines are used should be driven by rigorous scientific evidence, patient safety, and independent medical and public health expertise. Altering a proven vaccination schedule without credible evidence risks weakening public confidence and putting children’s health at risk. If you have questions about vaccines, talk to your child’s physician. Putting our children’s health at risk is not an option Read our full statement: spr.ly/6011B1KUUS

I’m a doctor. This executive order is wrong. The President does not have the expertise to make these changes. Vaccines are overwhelmingly safe. Vaccines are effective. Vaccines DO NOT cause autism. Breaking up vaccines will mean children have to get more shots to get the same protection, not fewer shots. It will increase hesitancy and make children less safe. Parents should listen to their child’s pediatrician about vaccines rather than listening to an inaccurate executive order. This is so wrong. nitter.cf/axios/status/208688294

Trump: "At one year, you should have 5 separate visits for vaccines rather than getting them all in the same day. I've seen proof of where they have a vaccination thing that looks like the size of a bottle of soda and pour it into a little child's body."

Much of the current outrage treats CPT as if the AMA invented it to game Medicare, but the facts does not support that assertion. The AMA published the first edition of CPT in 1966, the same year Medicare and Medicaid launched, and that first edition was almost entirely surgical procedures aimed at standardizing operative documentation. There was no RUC, no RBRVS, and no federal payment system built around it yet. It was the federal government that came to CPT, not the other way around. HCFA adopted CPT for Medicare Part B billing in 1983 and extended it to Medicaid outpatient surgical billing in 1986, choosing to build on an existing, functioning private standard rather than create a new one from scratch. The Health Insurance Portability and Accountability Act of 1996 went further, directing HHS to name national standards for electronic health transactions, and HHS designated CPT as that standard in 2000. Three separate federal decisions, spanning administrations of both parties, chose to keep using CPT. The governance and makeup of the committees are also designed to ensure a fair process. The CPT Editorial Panel, which writes and revises the codes themselves, and the RUC, which values the physician work behind those codes, are two different committees with two different memberships. The Editorial Panel's physician seats are nominated by national medical specialty societies, and its other seats go to the Blue Cross Blue Shield Association, America's Health Insurance Plans, the American Hospital Association, and CMS itself, none of whom are part of the AMA. RUC members, separately, operate under AMA structure and function rules that require them to exercise independent judgment rather than advocate for their own specialty's financial interest when a code comes up for review. No matter your opinions on the ultimate output of the committees, the claim that one trade association secretly runs both the code book and the price tag with no outside check does not match how these two committees are actually built.

So once again my wonderful insurance company, and its PBM partner, @CVS is blocking a medicine I have been taking for over 5 years. Instead of just renewing it, pre authorization has held it up for over 3 weeks in the hope I will just pay for it out of pocket.
