Excited to share the early feasibility and dosimetric results from our prospective trial of 5 fraction post-prostatectomy SBRT. CBCT based online adaptive treatment facilitated use of a modified reduced volume CTV and PTV (3 mm) w/many pts getting up LN and simultaneous boosts.

☢️Single-center phase II DAPPER study: 50 patients with biochemical recurrence after prostatectomy received CT-based daily adaptive SABR to the prostate fossa ± pelvic nodes (5 fx).
👉 Feasibility: 98% of fractions (246/250).
👉 Plan selection: Adaptive plans selected in 89% of fractions.
👉 Dosimetric outcomes: Improved target coverage and reduce bowel and rectal V25Gy.

Many reasons local rad onc centers are closing, but declining Medicare reimbursement is likely #1. Over the past decade, payment cuts have been ~2x greater for freestanding centers than hospitals. Many freestanding centers serve rural communities, impacting local access.
A new study in the #RedJournal found that 50 million Americans live in counties without a radiation oncology clinic.
As senior study author Kunal K. Sindhu, MD, explains: "The communities most vulnerable to losing radiation oncology access are often the same communities that already have fewer healthcare resources."
