(4/5) Finally, when decongesting overloaded patients, use a multimodal diuretic strategy (loop + thiazide +- acetazolamide).
Furosemide monotherapy will produce elevated HCO3, Na and low K. This is NOT intravascular hypovolemia but a common side effect of this medication. Using thiazide +- acetazolamide proactively will keep metabolics in tact - diurese to clinical markers of congestion/overload.
