….as their GP with no ability to limit demand or turn patients away. My colleagues are deflated/disgusted/resigned to it/working out their exit strategy. I hope @LSCICB act as they pay the Trust for all they do. Copying in @YvetteCooperMP @DHSCgovuk & @pulsetoday -it’s a new low

So my local FT - @UHMBT - has allegedly just told all its consultants they CANNOT book any further follow up appts for patients in their clinics. All clinics are now for ‘new’ patients. It’s an absolute outrage. What do patients do now for ongoing care. Yup - they come to us…..
A really interesting article however I really do think we need to separate out primary care and General Practice. They are not the same thing, and are often conflated to the detriment of patients.

Not so long ago a new frailty service would have been a local enhanced service issued with little fanfare. Now it’s a shiny new neighbourhood thing. Left trickle is not going to be the fundamental shift needed. It’s not system change it’s just working one pathway better.

GPs locally are seeing patients and signposting them to A&E. The GP front door service reassesses them and then signposts them back to the GP for them to do urgent bloods/ECGs etc etc.
Then those same GPs want jobs as salaried. Nope. You carry on dancing with the devil.

Being assessed by an expert generalist, as requiring acute care is a strong indicator that a person requires acute care.
We did an analysis a couple of years ago and found no value is reassessment of GP referral into acute settings.
If you consider it, a GP has thousands of examination experiences, so spotting the cases that require further investigation successfully shouldn't be a surprise.
I love the internet. So much.

Dear @bot, I wrote you, but you still ain’t calling. I left my @, my email, and my cell phone at the bottom. I sent two tweets back in autumn, you must not’ve got ‘em. There probably was a problem at the data center or something. Sometimes I type addresses too sloppy when I text ‘em. But anyways, what’s been up, man? How’s your founder? My email’s gon cold. I’m wondering if you can help me set up a new one at all.
We're gonna need a bigger boat. 🦈

Why do you have to degrade another profession to make your point?
“Skilled nurses” and “Junior Doctors” - two VERY different professions not doing the same job
Are junior nurses also 100% replaceable then?
“Junior” doctors aren’t replaceable - rotational training has shown that
For any GP Partners who missed out on the September dates....we are doing this again.

Time to tell a story...
End of last year?
Raised issues of patient safety and decisions made without discussing with primary care or those living with #Diabetes
Pointed out gaps in service, safety concerns
As night follows day?
Allegations and complaints made- designed to shut all raising of issues down.
Against one of the national leads in #Diabetes
Raising issues about what was designed & created by myself.
Rolled on to formal investigation.
Timeline from start? 8 months
Through personal grief- through all things.
Without much respite.
Yesterday? It ended- due to lack of proof- and no case to answer
Which was always the outcome - yet as ever?
It's the process that's designed to destroy.
Kept smiling - as I know the process well enough now.But heck-it's needed grit -especially since May
Am still here.
Still battling for better #Diabetes care
As I always say? You cannot shut me down.
H/T to @TheBMA for support
The next phase...is the next phase.
For better care of those living with #Diabetes?
There is very little I won't do.
And I haven't even mentioned the incredible bit about the difference in approach to others who have raised same issues- and coincidentally (of course) white.
Am still here.
Still here.

GPs add their greatest value when they confidently and correctly decide that NO ACTION is required based on experience that can’t be taught, it can only be earned.
The psychological harm from a 2WW breast referral at the 3% threshold is measurable. @DrNeenaJha
Hey Dispensing GPs and Pharmacists, anyone else noticed the change in the paper being used which is slowing down signing?
Shouldn't there be some kind of consultation before this is brought in? Or is it just me?
No one is bothered when it is administrators who are rejecting suspected Cancer referrals from GPs. Repeatedly. When we raise it formally we don't even get a response - unless you count the same individual rejecting it again.
It's like shouting into the abyss.

When ‘patients died’ becomes ‘blemishes on his record’, you’ve already lost sight of what the record is supposed to be about
This is how NHS execs cut services in full view. HSJ previously suggested that the QEH maternity service was at risk. There has been a drip drip of stories alleging terrible services and departments closing. This feels so co-ordinated.
Morale is rock bottom as a result.
Once a decision has been made I don't believe any campaigning will change it.
They are also saying with a straight face that they want people to have care closer to their home whilst discussing behind the publics backs the cutting of time dependent specialist services.
It definitely is a "...really thorny issue".
Recently I as the patients GP was asked to provide a copy of the Colonoscopy report that was done at the local hospital by the consultant that had carried it out.
It was apparantly easier to ask me to do this rather than them looking themselves at their own records.
#Thirlwall Total pie in the sky about interoperability of #NHS IT systems - ideal, necessary, desirable, Yes, possible in that time frame? Absolutely not.
