🩺 We Can’t Be Reactive — Dr. Tony Leachon Every typhoon season, the same story unfolds: flooded streets, overwhelmed hospitals, and families wading through contaminated waters. Leptospirosis cases surge, and we scramble — again — to respond. But public health cannot be reactive. It must be preemptive, physician-guided, and community-deployed. The idea of doxycycline prophylaxis did not begin in the Philippines. It was born from war. • During the Vietnam War (1960s–1970s), U.S. soldiers battling in jungles faced leptospirosis from floodwater exposure. • In Panama (1982), a landmark trial among 940 soldiers showed that weekly doxycycline reduced infection by 95%. • The study, published in the New England Journal of Medicine (1984), became the foundation for global preventive protocols — later endorsed by WHO and CDC for short-term, high-risk exposures. Today, the Philippine Society for Microbiology and Infectious Diseases (PSMID) and PhilHealth guidelines echo that legacy: • Single accidental exposure → 200 mg within 24–72 hours. • Continuous high-risk exposure → 200 mg once weekly. • Moderate risk with wounds or ingestion → 200 mg daily for 3–5 days under medical supervision. (Not for pregnant women, lactating mothers, or children under 8.) But how do we reach people when floods isolate communities? We must deputize barangay health workers, pre-position doxycycline in disaster kits, and include it in pre-typhoon checklists — alongside blankets, food packs, and flashlights. This is out-of-the-box thinking rooted in science and compassion. When doctors can’t reach flooded areas, trained local health workers can. When hospitals are full, prevention must begin at home. Leptospirosis is not just a medical problem — it is a systems problem. And the solution begins long before the floodwaters rise.
