It can happen while he's sleeping, playing football, sitting at home, or doing absolutely nothing unusual. It's called testicular torsion, and knowing what it looks like could make the difference between saving and losing a testicle. 1. WHAT EXACTLY IS TESTICULAR TORSION? Your testicles receive blood through structures called spermatic cords. These cords contain blood vessels, nerves and the vas deferens, which transports sperm. In testicular torsion, a testicle rotates and twists its spermatic cord, cutting off or severely reducing its blood supply. Without blood, testicular tissue begins to suffer damage. If circulation isn't restored quickly, the testicle may become permanently damaged or die. 2. WHY DOES IT HAPPEN? Some males are born with an anatomical variation called the bell-clapper deformity. Normally, the testicle is anchored within the scrotum in a way that limits its movement. With this deformity, it hangs more freely, making it easier to rotate and twist around the spermatic cord. This predisposition is often present in both testicles. Torsion can occur spontaneously, during physical activity, or even while sleeping. 3. WHAT DOES IT FEEL LIKE? The classic warning signs include: • Sudden, severe pain in one testicle. • Swelling of the affected side of the scrotum. • One testicle sitting unusually high or lying at a different angle. • Nausea or vomiting. • Lower abdominal pain. Not everyone experiences every symptom. 4. WHY THE FIRST 4–6 HOURS MATTER SO MUCH When the spermatic cord twists, blood supply becomes compromised. The longer the testicle remains without adequate circulation, the greater the risk of irreversible tissue damage. Surgical intervention within approximately 4–6 hours offers the best chance of saving the testicle. The chances of successful salvage generally decline as time passes. But 6 hours is NOT a cutoff after which treatment becomes pointless. 5. WHAT SHOULD YOU DO IF IT HAPPENS? If you experience sudden, severe testicular pain: • Go to an emergency department immediately. • Do not wait for the pain to disappear. • Do not sleep it off or assume it will resolve on its own. • Do not attempt to twist the testicle back yourself. • Do not delay emergency evaluation while arranging a routine appointment or waiting for an ultrasound. 6. ONE MORE THING EVERY YOUNG MAN SHOULD KNOW Sometimes, the testicle twists and then spontaneously untwists. The pain may suddenly disappear, making it seem as though everything is fine. This is called intermittent testicular torsion. But the disappearance of pain does NOT mean the underlying problem has disappeared. It can happen again, potentially with a longer-lasting episode that permanently damages the testicle. Sudden testicular pain that resolves on its own still requires urgent medical evaluation.
