In STEMI patients with multivessel disease, reduced LVEF increases ischaemic risk — but the COMPLETE trial showed complete revascularisation reduces major CV events regardless of LVEF.

👉PCSK9 gene editing: One shot. Lifelong LDL-C lowering?
☝️ A new review explores the promise and challenges of CRISPR-based PCSK9 gene editing.
1️⃣ One-time treatment: A single infusion could provide lifelong reductions in PCSK9 expression and LDL-C.
2️⃣ Early human evidence: VERVE-102 and YOLT-101 have demonstrated substantial LDL-C reductions, reaching approximately 50% at selected doses.
3️⃣ The major advantage: Decades of LDL-C lowering without repeated dosing. A potentially transformative approach to cumulative cholesterol exposure.
4️⃣ Safety considerations: Early clinical results are encouraging. Larger studies and longer follow-up are needed to establish long-term safety and durability.
☝️ The next frontier: Can sustained LDL-C reduction from a single intervention translate into greater lifetime cardiovascular protection?

Transcatheter vacuum-assisted extraction of left heart and aortic arch masses showed promising early effectiveness in this multicentre registry.
The observed rate of adverse events suggests the need for careful patient selection and improved procedural safety.

When Is a Coronary Artery “Small”?
RCTs suggest DCBs are non-inferior to DES in small vessels. But where’s the cutoff?
The magic number for long-term stent patency is an MSA of 5.5 mm², corresponding to a vessel diameter of 2.6 mm. See the sizing chart below.
The question of the day: Is a ref. vessel dia. (RVD) of 2.75 mm the tipping point for choosing DES over DCB? 🤔
Replying to @Hragy @realarainmd @mmamas1973 @mirvatalasnag @ShariqShamimMD @AntoniousAttall @aymanka @TWilsonMD @jedicath @NishithChandra
If 3mm DCB fully yielded without recoil in this young man without heavy calcium then that’s an MLA of 7mm2. It’s adequate.

Thoughts on How to Achieve the Optimal MSA with PCI
The suggested cut-off of 2.75 mm in my previous post is to guide the choice between DCBs and DES.
It is NOT a suggested target MSA. The goal is to achieve at least 90% of the ref. MSA, as proposed by @SripalBangalore et al.
Additional thoughts on how to do this are in my reply.
Replying to @realarainmd @agtruesdell @jedicath @Hragy @mirvatalasnag @rajivxgulati @stefan_harb @ihtanboga @melsharabasssy @DocSavageTJU
Interesting way of looking. There are obvious limitations of absolute MSA threshold. A MSA of 5.5 in a proximal artery with ref dia of 4.0 mm, is only 44% area expansion but clearly too big for distal small vessels. In an editorial, we proposed (for LM) the below relative MSA.
