Obesity innovation risks widening health inequalities if access is unequal.
Low adoption and disparities in use likely reflect system-level access barriers
In >39 million US adults eligible for obesity treatment, only 2.3% received semaglutide/tirzepatide.
Prescribing was lower in:
📍 Most socially vulnerable: 1.9% vs 2.6%
🏡 Rural: 1.5% vs 2.4%
🌍 Hispanic, Asian & Black populations vs White populations
Effective treatments only improve population health if those most in need can access them.

The next stages of #T1Diabetes -and the advance of science.
It took us a decade (2016-2026) to bring #T1D care to be one of the best around.
Early signs of the #NHS starting to lose ground- as little to no work evolving in this area at the moment.
Sporadic, fractured and scant leadership.
A discussion even to consider whether the NHS should do it, afford it etc needs to start.
Thankfully- technology access in #T1D can't be reversed.
But keeping up with changing science is imperative.
And am afraid the signs are we are not good.
Also...folks...start thinking around 'Islet Cell Transplants' and the future around it- now.
It takes a long time to catch up without the infrastructure

What explains semaglutide’s 20% reduction in MACE in SELECT?
Not simply weight loss.
Potential mediation signals included:
📏 Waist circumference: 64%
🧪 hsCRP: 42%
🩸 HbA1c: 29%
⚖️ Body weight: 20%
But confidence intervals were wide, and known risk factors could not fully explain the cardiovascular benefit.
The mechanism appears broader than weight loss alone.

Overweight/obesity and psychological distress don’t just coexist
..together they may amplify the risk of long-term sickness absence.
In nearly 4,000 working-age adults:
📈 Short Sickness Absence : RR 1.62
📈 Long Sickness Absence : RR 2.48
⚠️ Evidence of synergy for longer absences
A reminder that obesity care must address mental as well as physical health.

Thanks - great to see this published as an output from the @REDDIE_Diabetes project, a "fit-for-purpose" tool to help identify the right RWE database to answer your specific question will follow soon @ABCDiab @EASDnews @JonahTh0mas @ShivaniM_KC
Only 22 of 610 diabetes real-world data databases were from lower- and middle-income countries.
Our global review led by @drpratikc @JonahTh0mas @MadderJules
🌍 610 databases across 74 countries
🇪🇺 193 in Europe
📊 Demographics, prescriptions and biochemistry were commonly captured
📉 Mortality and CGM data were much less available
🔓 Very few datasets were open access
A new online dashboard maps the global diabetes RWD landscape but highlights a major gap in global representation.

Diabetic foot ulcers are more than a wound.
Our study led by Molly Caba and @MichelleHadji
DFUs can affect mobility, weight, mood, mental health and future cardiovascular risk.
Interviews with people living with T2D and DFUs highlighted major unmet needs:
🧑🤝🧑 shared decision-making
🦶 better physical support
🧠 access to psychological care
DFU pathways need to be truly holistic.
