🧬 "Qi" Isn't a Biomarker. It's the Relationship Between Immune and Mitochondrial Biomarkers. 🧬 This may be the most important thing I ever post. My biggest contribution to modern medicine may simply be mainstreaming the Chinese medicine concept of "Zheng Qi": mitochondrial function and immune competence are not two separate systems. They are directly coupled and should be studied that way. We already know this biologically. MAVS - mitochondrial antiviral signaling protein - is literally anchored to the outer mitochondrial membrane and helps launch the cell's antiviral response; it's the only protein downstream of RIG-I that bridges antiviral immunity and energy metabolism. Immune cells also change how they make energy depending on what they need to do - effector cells run on glycolysis, regulatory and memory cells on OXPHOS - while chronic immune activation damages mitochondrial function in return, through PD-1/CTLA-4 signalling that causes depolarization, defective OXPHOS, and ROS-driven mtDNA damage. And when mitochondria fail, they leak mtDNA into the cytosol, where it acts as a danger signal that drives chronic inflammation. The literature names it explicitly: "The relationship between mitochondrial dysfunction and immune activation has been recognized as a bidirectional feedback loop." In other words: energy production shapes immune competence, and immune activity shapes energy production. Chinese medicine has had a systems-level concept for this relationship for centuries. Zheng Qi describes the body's integrated capacity to make energy, defend itself, clear pathogens, repair damage, and recover. This matters enormously for Long Covid, ME, and other chronic energy-limiting or immunological illnesses, where mitochondrial and immune abnormalities repeatedly appear together. But it matters far beyond those - the same coupled system is now documented in Parkinson's disease and neurodegeneration, in autoimmune rheumatic disease (RA, SLE, systemic sclerosis, where mitochondrial dysfunction and glycolytic bias converge as a core mechanism of chronic inflammation), and in cancer and metabolic disease, where immunometabolism is the shared substrate. This isn't a niche observation about fatigue. It's a principle that runs through all chronic illness. Qi isn't a biomarker. It's the relationship between the biomarkers. If modern medicine takes one thing from me and Chinese medicine, I hope it is this: stop studying energy failure and immune failure as separate problems. I’m looking for something. In 1971, James C. Gaither, who served as general counsel to RAND corporation, wrote a post-mortem report detailing the theft of the β€œPentagon Papers” from their Santa Monica facility. It was circulated to the RAND board. I’ve spoken to Gaither, and he does not have a copy. RAND will not disclose whether they have a copy or not. Autumn is Vashti Bunyan season. She is one of the most fascinating, & neglected, musicians currently living. In 1968, she and her then-boyfriend, also a musician, set out on a two-year journey across Britain sans car, accompanied by her horse Bess. Winters were cold, and their attempts to play music on the village greens were invariably broken up by the police (villagers thought Vashti was trying to kidnap their children). They traversed the Scottish Highlands and ate porridge in cobweb-shrouded cottages on remote islands. And as they traveled, Vashti wrote songs to pass the time. And before long those songs came to the attention of others, who informed her that she had a remarkable gift. An album was recorded in 1969, Just Another Diamond Day, featuring fourteen perfect songs. It’s now recognized as one of the great albums of the twentieth century, maybe of all time. But in 1970 it sold only a few hundred copies, sending Vashti into a depressive spiral. She abandoned music entirely for several decades, until one day in the mid-1990s she googled herself and learned that she had become famous. Her songs were beloved by a new generation of artists, including Joanna Newsom and Devendra Banhart, who fought for the re-release of her early work. She embarked on a global tour and began writing again. As Rob Young says in his book Electric Eden, β€œShe had picked up the pilgrim trail once again and found the niche success she had sought as a teenage songwriter.” Seldom has belated fame been more deserved.

A personal long COVID update: I'm doing really well! Five years ago, I was largely confined to bed. These days, I'm more active, even traveling with my family. I still respect my limits, but the difference is remarkable. I feel great, and I’m enjoying every minute of it.
