Thanks @charlie_lees . IMHO all combo therapies ( or for that matter new mono 🤔 ) trials should have an adjunct dietary intervention arm
Thoughts ? 💭
NEW ON ATOMIC IBD (link to full article below)
Eight years ago I had the idea of combining ustekinumab and vedolizumab after anti-TNF failure in Crohn's disease. No one would have funded it then.
Now J&J, AbbVie, Spyre, Lilly and Takeda are all testing versions of that idea. Part 1 of my combination therapy series is up on Atomic IBD, and it asks the obvious question: do two advanced therapies beat one?
Every estimate so far leans towards the combination (image 1 below). But no combination of two advanced therapies has yet beaten the best single drug on a primary or multiplicity-controlled endpoint, across a whole trial population, in a blinded randomised trial.
The gains that clear zero come in the most refractory patients. In DUET-CD, after two or more failed therapy classes, 49% were in clinical remission at week 48 on high-dose JNJ-4804 v 27% on guselkumab, in an exploratory subgroup. In TARGET-CD, endoscopic remission at week 24 was 41% on ABBV-466 v 22% and 20% on each drug alone, open label.
The piece also covers why a two-in-one injection is hard to make and to prove, which pairs are being tested and why, whether two biosimilars could take on the branded combinations, and four different ways to combine.
Only three of these trials are in phase 3 (image 2 below). VICTRIVA reads out in 2027, then COMMIT and DUET ENCORE, the first phase 3 of a two-in-one injection, in 2028.
Part 2 turns to oral combinations, including whether an IBD polypill is realistic.

1/10 🧬 B cells in IBD: mucosal protectors, disease drivers, or both?
Dr. Saurabh Mehandru's Cleveland Clinic lecture explores the promise and pitfalls of B-cell targeting. Despite an expanding treatment toolbox, we still need a deeper understanding of disease biology to achieve durable remission. 🔬 #IBD
This is in response to breaking the therapeutic ceiling effect with current IBD drug landscape

Excited to share our newly published narrative review in Intestinal Failure! 🧬📚
We explore predictors, risk factors, and management strategies for intestinal failure in Crohn’s disease, highlighting the importance of early disease control, bowel-sparing surgery, and perioperative optimization to prevent this devastating complication.
Grateful to my co-authors and mentors for making this work possible! 🍀

Great feedback about this session particularly from those who were not able to go to 🇺🇸 this year - thank you . Cross organisational collaborative sessions are needed given the pace of read outs of trials
Thank you also for putting MINIMISE on the list 😊
