Classic Barlow valve with mixomatous degeneration as shown by 2D/3D TOE:
✔ Huge annulus dilatation (48 mm - intercommissural)
✔ Main lesion: wide P2 prolapse, small P3 prolapse
✔ Secondary lesion: A1 prolapse
✔ Mitro-aortic angle: 110°
✔ SAM-risk: ++++
Surgical approach in right mini thoracotomy:
✔ P2 resection
✔ A1 neochordae implantation
✔ Complete annuloplasty (38 mm ring)
✅ No residual MR
✅ No SAM

70 y/o with NICMP, afib on eliquis presents with showering all over (black toes,.. etc), with very large LAA/LA mass. Successful AlphaVac (1st in the state of MI of using AlphaVac for left heart). Prohibitive groin thus AlphaVac. Alpha vs angiovac for left heart? Thoughts?
