This is exactly the issue. Thirlwall has made 17 recommendations — but recommendations only protect patients if somebody makes sure they are actually implemented. We have had inquiry after inquiry, review after review, identifying failures and recommending change. The real question is: who tracks delivery, who challenges organisations that fail to act, and who is held accountable when recommendations are ignored? The government now says there will be a single tracker for recommendations from major maternity and neonatal inquiries. That principle needs to go much wider across healthcare. Otherwise we risk continuing the same cycle: tragedy, inquiry, recommendations — and then another family asking why the lessons were never learned. #PatientSafety #Accountability #PublicInquiry #MatthewsCampaign
