Why I keep my degree in my twitter title: 1. Dr. can be anything from a PhD in English to a physiotherapist,I like to keep things clear. 2. As a 1st gen doc,it represents generations of dreams & hopes. 3. It took a lot to earn them, and I want to flaunt it. Go on,Judge me!

The "work life balance" in anesthesia comes much later if you want when you've already learned and seen everything. At this stage, to learn how a case is supposed to be done and how a particular comorbidity / physiology is supposed to be managed, you need to actually sit in OT.

#DidYouKnow Durga Puja in Kataka has a unique fish bhog tradition on Maha Navami! Today, at Chandni Chowk and Alisha Bazaar, around 20 quintals of fish being offered to Maa Durga during Navami and distributed among thousands of devotees. At Chandni Chowk, this tradition dates back to 1817! The traditional bhog also features khata and kanika, served alongside fish bhog.

Thanks @AirwayMxAcademy , I do appreciate there's no evidence that gastric US changes outcomes. That said, there are lots of things we use every day that provide useful information while lacking evidence. Pulse oximetry has not been shown to reduce periop mortality, but I sure ain't gonna stop using it while I await some dude to do a meta-analysis on it. Here's the real value: In the above example, if I scan and find stomach is empty, that means I can proceed confidently with my plan of spinal plus propofol sedation (aka unconsciousness/loss of airway reflexes). If I were just to reflexively intubate the tracheas of every joint replacement on a GLP1 (about 15-20% now), I would now be exposing those patients -- as you point out -- to a new set of risks (airway trauma, bronchospasm, blah blah blah). If I DO find a full stomach, then we have some decisions to make. Spinal with the patient awake through the procedure is one option. Postpone is another. Re-scan an hour or two later is a third. GA with RSI is an option I suppose BUT....RSI is hardly a guarantee of safety. Most operative aspiration events occur during RSI/full stomach. Here's a question for everyone out there (and I'm genuinely curious). If you have a known full stomach and you proceed with GA/ETT for an elective case and the patient has a serious aspiration event, is that defensible? I reaaaallllyyy hate questions that start with "Dr. Gadsden, please explain to the jury ..."

It is not so black and white. Suxamethonium still has a place in clinical practise. I use it for the following indications: 📌quick and safe treatment of laryngospasm 📌reliable relaxation if supraglottic airway device does not work but desaturation already starting 📌intralingual injection when IV stops working during induction 📌ECT procedures 📌short ENT procedures 📌rocuronium allergy 📌airway rescue 📌procedures where surgeons use nerve stimulators 📌complex airway but sugammadex not available #suxamethonium #suxisnotdead @DaveOlvera1 @jducanto @DrHuw

Given the high short term mortality risk in this particular scenario, I would also discuss non-operative management combined with a phenol nerve block with the patient and her family. pubmed.ncbi.nlm.nih.gov/4271…

So when you are in India and suffer from the lack of a USG(look up the PCPNDT Act,before you come at me),you can still use landmark technique do the DEEP Serratus Anterior Plane Block, that's why it wins for me! #MedTwitter
There are two different versions (of course there are…*eyeroll emoji) A superficial and a deep approach, where the local is placed above and below the serratus muscle, respectively. I have my thoughts on which one I like, but how about you? #Blocktober26

Two reasons why October is my favourite month of the year; 1)it's my birthday month 2) it's #Blocktober26 😁😁😁 @jeffgadsden
Good morning and welcome to #Blocktober26 day two! You’re working with a new shoulder surgeon who likes his patients to be awake throughout. Cool! You perform superior trunk block for arthroscopic rotator cuff repair with 10 ml of 0.5% ropivacaine. Twenty minutes later, the patient can barely lift their arm and the shoulder feels numb. Sweet. BUT...when the surgeon starts work on the port site on the posterior shoulder, the patient nearly jumps off the bed. Bummer dude 🫤 What happened—and what are you going to do about it? 🧵👇

What started as a dream has now become a reality! The dream of establishing the first ATLS Provider Center in Northeast India at AIIMS, Guwahati began with our own training, followed by successfully clearing the site inspection. Since then, there has been no looking back! We have successfully conducted three courses on 12–14 Dec 2025, 13–15 Mar 2026, and 7–9 Aug 2026, and are now geared up for our 4th course on 29–31 Oct 2026. Forever grateful to Prof. Ashok Puranik, executive director, AIIMS, Guwahati, Prof. M. C. Mishra, ATLS Chair, India, Prof. Amita Ray, Site Inspector and all our external faculty and delegates for their invaluable support. A special cheer to our in-house faculty, site coordinator, and supporting staff for their unwavering dedication and teamwork in making every course a success. A dream achieved. A journey in progress. A commitment to saving lives. Long live ATLS! 🚑🩺 Long live AIIMS, Guwahati @AIIMS_Guwahati @ATLS_India – at Guwahati, India

Kya ho raha hai yeh 😥 suicide is not an answer right? If you're stressed out, take a break, at least 2 weeks. Go home, agar leave nai de rahe toh let them extend your residency, cut salary or whatever. Suicide is not an option 🙏🏾

We will do anything in India to avoid discussing mental health in India,be it celebrity deaths or student deaths. Conspiracy to casteism everything is valid EXCEPT mental health struggles. This attitude is what is truly DISGRACEFUL!
