One of the most common questions I get from referring urologists: "Is HoLEP safe for patients in acute urinary retention who are catheter-dependent?"
This new study of 140 patients answers clearly: yes. Complication rates were comparable between AUR and non-AUR groups. But what stands out is how much more AUR patients improve — greater gains in Qmax, IPSS, quality of life, and post-void residual.
This matches with my experience. Catheter-dependent patients often arrive discouraged, depressed, having been told surgery is too risky at their age or with their comorbidities. In reality, these are often the patients who benefit the most. The change between their pre-op misery and post-op quality of life is dramatic.
The key finding: although AUR patients showed more pronounced improvement, final postoperative outcomes were statistically equivalent. HoLEP returns function regardless of baseline severity.
For urologists still hesitant to offer HoLEP to catheterized patients: the evidence continues to support it. Do not let a catheter delay the definitive solution.
