Convert a continuous variable into a binary variable (hi/low) loses info unless n goes up
" in the most favorable case, the loss of information due to dichotomization would have to be compensated for by an increase in sample size by a factor = 1.57".

The Department of @FishOceansCAN is considering removing the principle that salmon are a public resource managed for all Canadians.
This would be a colossal shift in conservation law and public access.
If adopted, fishing would no longer be a shared right but a "privilege" granted by First Nations, placing non-Indigenous Canadians last.

🤔 Do continuous SPIP catheters improve sternotomy pain
🚀 A new RCT in RAPM found no difference in pain, opioid use, or recovery vs single-injection SPIP blocks after cardiac surgery. Evidence > assumption!

A seminal paper dismantling 50 years of critical care dogma and RCTs failing patients
@PatientStormDoc publishes a historical review revealing that decades of critical care trials for sepsis and ARDS rely on the flawed "Petty-Bone" design, which substitutes synthetic, threshold-based syndromes for distinct biological diseases.
Using Causal Symbolic Modeling and Directed Acyclic Graphs, the author demonstrates that these synthetic surrogates function as cohort colliders, mixing incompatible pathologies like viral and bacterial pneumonia into unstable groups.
This conceptual error renders trial results non-transportable, explaining replication crisis in critical care RCTs and high-profile reversals after causing patient harm.
The study exposes how international task forces have perpetuated a "pathological consensus" by recycling arbitrary surrogates rather than addressing the fundamental lack of causal validity in their designs.
The author argues we must abandon the Petty-Bone RCT framework immediately to end this era of "science mimicry"—and mandate causal modeling in all future grant-funded research and CONSORT guidelines.

Cardiac surgery is often viewed as a disposition for the sicker, terminally ill patients rather than a therapeutic viable option by many non-surgical colleagues. This is one of the biggest threats to our field as a whole, but never raised as a concern by our leadership.

I watched a mock interview recently where a student was asked about their greatest strength.
They told a dramatic story about catching a rare diagnosis that the attending missed. They thought they looked brilliant.
The feedback they got?
“Unbelievable and arrogant.”
Here is the hard truth about residency interviews: PDs aren’t hiring you to be House M.D. on Day 1.
They are hiring you to be the person who doesn’t fall asleep when the pager goes off at 3 AM.
Stop trying to prove you are a genius.
Start proving you are high-reliability.
Most applicants use the “Hero Framework”:
I saw a disaster → I saved the day → I am amazing.
This fails because it highlights luck, not skill.
Instead, use the “Micro-Trust Method.”
This framework proves you can handle the unglamorous, grinding reality of residency.
Here’s how to structure it:

In this analysis of >1,000 TAVI patients, we demonstrate that underexpansion of balloon-expandable transcatheter aortic valves, as assessed by fluoroscopy, is associated with an increased risk of hemodynamic valve deterioration at mid-term follow-up.

Then I got even more excited because it used the wearable cardiac defibrillator and VEST trial as an example.

🚨More rehospitalizations (x2) after TAVR in EARLY TAVR (26%) than in PARTNER 3 (13%) at 5y— despite asymptomatic patients and similar STS risk? Slightly higher age in EARLY TAVR is no excuse.
If the idea was to intervene earlier, it’s hard to see where the benefit lies.
