Honey Buys Minutes, Not Safety ————————————— A 2-year-old who swallowed a button battery 30 minutes ago can look completely well and still be in serious danger. That is the trap. Lithium coin cells generate an alkaline current the moment they contact moist tissue. Serious esophageal injury can begin within minutes and become catastrophic in as little as two hours—even while the child swallows normally and has no pain, drooling, or vomiting. The correct first move is B: give honey on the way to the ER. If the child is ≥12 months, the ingestion is within 12 hours, honey is immediately available, and the child can swallow, give 10 mL (2 teaspoons) of commercial honey every 10 minutes, up to six doses. Honey is viscous and mildly acidic. It coats the battery and slows local hydroxide generation, reducing the depth of alkaline burns. It is first aid, not treatment. Do not induce vomiting—that can cause more trauma as the battery moves. Do not give milk or other drinks; they lack the same protective effect and can complicate later endoscopy. Do not delay leaving to find honey. The child still needs an urgent X-ray. An asymptomatic toddler can have an esophageal impaction that requires immediate endoscopic removal. Honey buys a little time. It does not make the battery safe. Call poison control or the battery hotline, start honey if criteria are met, and go now.
