Alzheimer’s disease can begin changing the brain years before symptoms appear. Understanding what drives those changes, and intervening earlier, could open new opportunities to prevent or delay cognitive decline. @ARPA_H is investing up to $48.8 million in new Alzheimer’s research focused on that early window.
Mass General Brigham is launching a national clinical trial platform to test multiple preventive therapies simultaneously, before symptoms emerge. Washington University in St. Louis is building an open, AI-enabled data platform to help uncover the biological drivers of the disease. Other teams will investigate the earliest stages of cognitive decline and study how interventions affect brain health in at-risk older adults.
These projects build on a broader ARPA-H Alzheimer’s research portfolio totaling up to $155.3 million. It includes BIOGAMI, which aims to detect and address early protein dysfunction, and NIGHT, which is studying ways to improve sleep and mitigate women’s elevated risk of Alzheimer’s.

At A4LI, we believe advancing human health means addressing age-related diseases before they take hold. Research that uncovers the earliest drivers of Alzheimer’s could help shift the focus from managing disease to preventing or delaying its onset, giving more people the opportunity to live longer, healthier lives.
Women live longer than men on average, but those additional years are not always spent in good health. Closing that gap requires research that better accounts for women's biology and addresses the health challenges they face throughout life.
The Women's Health Executive Order calls for more research into conditions ranging from cardiovascular disease and Alzheimer's to osteoporosis, menopause, and aging. It directs the NIH to establish a dedicated Women's Health Division within 90 days and creates a White House Women's Health Task Force to coordinate efforts across federal agencies, including @NIH, @FDA, @CMSGov, and @ARPA_H.

At A4LI, we believe everyone should have the opportunity to live a longer, healthier life. Recognizing aging as a women's health priority is an important part of that goal. Continued investment in research and innovation can help ensure that longer lives also mean more years lived in good health.
Osteoarthritis affects tens of millions of Americans, and advanced cases can leave patients with few options beyond joint replacement. Current treatments can help manage pain and improve function, but regenerating lost cartilage and bone remains a major challenge.
@ARPA_H's Novel Innovations for Tissue Regeneration in Osteoarthritis (NITRO) program is working to change that. Research teams at Duke University, the University of Colorado Boulder, Columbia University, and other institutions are developing experimental approaches ranging from targeted drug delivery to 3D-printed, living joint scaffolds. In animal studies, some approaches have shown promising signs of cartilage regeneration. The goal is to move these innovations toward human trials while developing treatments that could cost substantially less than current standards of care.

At A4LI, we're encouraged to see ARPA-H invest in research that could change how we treat age-related conditions. Maintaining joint health is essential to mobility and independence, and regenerative approaches could one day offer patients options beyond managing symptoms or replacing damaged joints. We'll be following NITRO as these experimental treatments advance toward clinical test.
Pancreatic ductal adenocarcinoma is one of the deadliest and most treatment-resistant cancers. It is most often diagnosed in older adults, frequently at a late stage when options are limited.
@matterbio has received FDA clearance of its Investigational New Drug application for LM-LLO-TT, an immunotherapy built on a weakened form of the bacterium Listeria monocytogenes. It is designed to train the immune system to recognize and attack tumor cells. The clearance moves Matter Bio into the clinic with a first-in-human trial. The trial will test LM-LLO-TT alongside the chemotherapy gemcitabine in patients with unresectable pancreatic cancer, looking at safety, the right dose, and early signs of anti-tumor activity.

Diseases like Alzheimer's, Parkinson's, and ALS can develop over many years, with protein misfolding and clumping contributing to damage in the brain and nervous system. By the time symptoms appear, significant damage may already have occurred. Understanding how these changes begin could open new opportunities to detect and treat disease earlier.
@ARPA_H's new BIOGAMI program is tackling this challenge, investing up to $152 million over four years to understand how proteins become dysfunctional and develop ways to intervene. Six research teams, including those at the Buck Institute for Research on Aging, Johns Hopkins, MIT, and UC Berkeley, will combine protein science and computational tools to develop new approaches to early detection and treatment. While the initial focus is neurodegenerative disease, the findings could eventually have applications in cancer, diabetes, and other conditions.

Bringing a new drug to patients typically takes more than a decade and costs around $2 billion, and about 90% of candidates fail along the way. Much of that time and cost comes from a rigid, phase-by-phase clinical trial process that has changed little in decades.
@ARPA_H's new SURPASS program aims to change that. It uses digital twins, predictive modeling, and real-time data analysis so trials can adapt as evidence comes in, rather than waiting for each phase to finish. SURPASS launched alongside three other HHS efforts to expand clinical trial sites, build privacy-protected data infrastructure, and give patients a greater role in research. Together, these efforts could mean shorter timelines, lower costs, and fewer participants needed to reach clear answers.

At A4LI, we are proud to see ARPA-H take on one of the biggest bottlenecks in modern medicine. The faster we can generate reliable evidence, the sooner promising therapies for age-related disease can reach the people who need them. We will keep advocating for a research and regulatory system built for the speed of today's science, and we look forward to following SURPASS as it moves forward.
ARPA-H Advancing Clinical Trials Announcement nitter.cf/i/broadcasts/1vKpPNOwV…
Clinical trials still move like it’s 1995.
A decade of waiting, $1–2 billion spent, and a 90%+ failure rate on a clunky, fragmented system.
ARPA-H’s SURPASS program is rethinking the entire clinical trial model.
Adaptive platforms, predictive models, real-time analysis, and AI will make trials faster and cheaper so treatments reach patients years earlier.

Many cell-based therapies, including treatments for cancer and autoimmune disease, currently rely on storage at temperatures as low as -196°C. Maintaining that ultra-cold supply chain adds significant costs and can make these therapies harder to deliver where they are needed most.
@ARPA_H’s new BioStabilization Systems (BoSS) program is taking on that challenge, investing up to $87 million to develop technologies that could keep life-saving cell therapies stable at room temperature.
A 200-degree leap could mean more than eliminating freezers. It could help lower costs, simplify distribution, and make advanced therapies more accessible to patients and providers, including at community hospitals.
