#EMUC26 in Brussels: Proud to see Jonas Repkewitz, our former lead resident doctor, present our IMPACT-UK multicentre study on triplet therapy in APMN/mHSPC. An outstanding clinician, researcher and rising oncologist! Do stop by our poster! @BartsECMC @NHSBartsHealth

Surgery is SOC for operable early-stage NSCLC; VALOR may change that. Emulation can’t fully adjust for why SABR was chosen: often substantial comorbidity, frailty or limited physiologic reserve. Retrospective OPSCC studies reported better surgical OS; ORATOR didn’t @drdavidpalma
Replying to @BrendonStilesMD
The key finding: Adjusted 3-year all-cause mortality: 🔹 Surgery: 21.6% 🔹 SABR: 30.3% Absolute difference: +8.8 percentage points with SABR (95% CI 1.4–16.2; P=.02). The survival curves favor surgery. An important finding in a debate long short on randomized data.

Non-randomized phase II study of neoadjuvant ivonescimab in pts with resectable NSCLC @NatureMedicine. Ivonescimab alone (n=10): MPR 60%, pCR 30%. Ivo + chemo (n=67) MPR 70%, pCR 51% (61% in squamous). Only 1 treatment related surgery delay. nature.com/articles/s41591-0…

Another negative trial with concurrent RT and IO in NSCLC (this time in early stage I-II). Important to note that >80% were medically inoperable. Timing of IO with RT clearly matters here as with PACIFIC. For now SOC in medically operable patients is surgery until we have VALOR.

I captured this video in my lab to show the power of medicines called T-cell engagers. Left: T cells move in and out of a cancer tumoroid, largely ignoring it. Right: Add a T-cell engager, and the T cells attack. Once they recognize their targets, they tear through the tumor.

KRAScendo-1 #ESMO26 shows divarasib (more potent iKRAS-G12C) > sotorasib/adagrasib for PFS/OS. Active drug and new SOC IMO in this group. Whilst G3 AE lower with div, seems to be mainly driven by less transaminitis. GI tox is higher and proactive rather than reactive Rx is key.

SAFFRON #ESMO26 shows benefit in PFS/OS for adding MET targeted Rx into osi after 1L-PD. IMO it’s an additional option rather than the new SOC here. Busy space with MET-active (MARIPOSA, CHRYSALIS) and agnostic (COMPEL, TROPION, Opti-TROP) options. Re-biopsy is a limitation.

🚨 Practice-changing KRAS G12C NSCLC #ESMO26 Divarasib may be the most potent inhibitor yet 📈 PFS: 9.8 vs 5.7m (HR 0.58) 📈 OS: 17.7 vs 12.0m (HR 0.56) 🎯 ORR: 58% vs 32% Rapid 🇬🇧 reimbursement needed given challenges with sotorasib. 🔬 KRASCENDO-2 is OPEN at Barts!

New standard of care for KRAS G12C mutation positive NSCLC. @FSkoulidis presents Krascendo-1 head to head trial of divarasib vs sotorasib or adagrasib- convincing improving PFS (9.8 m vs 5.7 m, HR 0.58) and OS (17.7m vs 12.0m, HR 0.58)

Very well conducted study with hard to ignore results. Differences aren’t due to post-PD access. Definitely will change how we approach discussions with patients but I don’t think we can adopt a one-size-fits-all. Some patients still get excellent benefit from monotherapy.

Practice changing results from DeLLphi-305 #ESMO26 and UK NICE needs to quickly help make this available. Attrition post-1L is high so bringing tarla forwards makes sense. Minimal G3 CRS/ICANS probably from early cytoreduction of tumour. OP delivery possible (2 hour monitoring).

Impressive results from INTerpath-001 #ESMO26 showing personalized mRNA vaccine works. Extraordinary tech but 17% did not have enough tissue for development. No increase in immune-tox and G5 were prior to mRNA. Not sure how this fits with NADINA & S1801. Cost will be a barrier.

#ESMO26 | PERSEE – Phase III In advanced NSCLC with PD-L1 ≥50%, adding chemotherapy to pembrolizumab improved median PFS from 6.9 to 16.1 months (HR 0.56). The numerical OS difference was notable (50.2 vs 29.9 months; HR 0.77), but not statistically significant. However, we should remember that OS was not the primary endpoint! Grade ≥3 toxicity: 53.8% vs 19.3%. I think adding chemotherapy provides benefit, but at the cost of increased toxicity. We need to better identify the patients for whom this toxicity is worth it.

Once in a while I get to read an article so good that I hope the rest of the world also reads it. Go home, drink the good wine, and read this. Superb editorial from @Prof_IanD : ascopubs.org/doi/10.1200/OP-…
