My top 3 “most exciting” clinical observations from Day 1 of the MR-Linac Consortium:
1. Esophageal adenocarcinoma: Integrating adaptive MR-guided SBRT to gross disease after neoadjuvant FLOT appears feasible and safe. Two targets using an SIB approach: pre-treatment gross disease and residual gross disease. 3-mm PTV margins. Phase 1 outcomes are maturing.
2. Direct-to-Unit (DTU) workflows: DTU has expanded well beyond inpatient palliative care: prostate SBRT, GBM, liver metastases, and oligometastatic lymph nodes. This has the potential to dramatically increase access to MR-Linacs.
3. Early-stage glottic cancer: Twice-weekly SBRT (8.5 Gy × 5) appears feasible and safe. 2-mm PTV margins, except 4 mm CC. No CTV. The target can be tracked using the cricoid. Phase 1 outcomes are maturing.
Bonus:
3b. GBM: FLAIR changes that emerge during long-course RT may have prognostic and/or predictive value. Seen in ~5–10% of patients.

3m mFOLFIRINOX or Gem+nab-Paclitaxel alone +/- SBRT (40 Gy/5 fx)
Primary endpoint: locoregional recurrence
🚨SBRT had improved:
✅12 m LRR: 20% vs. 50%
✅mOS 22m vs. 14 m

👇As systemic therapy improves, local control matters more. We’re using RT w/ RASi on EXPAND and in trials upon oligoprogression. It's a new horizon for pancreatic cancer— pts benefit from evidence of added value to bring this opportunity to more. @PanCAN @utswcancer @UTSW_RadOnc

Enjoyed this session & learned a lot!
Both TARE & SBRT can deliver ablative RT in HCC. Key q is which approach is best suited for a given pt/tumor.
Sharing some of my slides below re: decision-making b/w the 2 modalities.
TY to @GIcancerDoc & @EileenMOReilly for the invite.

Thoughtful read on how oncology rewards innovation.
Much of RT progress = precise & efficient tx, improving therapeutic ratio but reducing reimbursement. Systemic advances often =drugs given for yrs.
Current model rewards quantity/duration, but shouldn’t value reflect benefit?
How do we think about the cost of systemic therapy agents as compared to other cancer treatment modalities such as SBRT when considering value and is it time for a change?

GI003 HCC proton vs photon: no dif OS, PFS, Gr 3+ AEs.
Protons = 1 option in expanding RT toolbox (MRgRT, adaptive, etc) to optimize therapeutic ratio, & each case should be individualized by tumor & liver fxn.
But w neg RCT & $/access dif, photon RT should remain SOC. #ASTRO26
