Shout out to all the middle agers still getting it! Being fit at 27 versus 47 are two different animals. While definitely more barriers as we age (physiology, responsibilities, periodic aches) it’s still doable n arguably even more important. Keep getting it!
Why do so many people become outraged when human adaptability is showcased at it finest? To the contrary, shouldn’t we be celebrating what we’re capable via methodical progressions? Too many movement police out there, not enough enthusiasts. We’re not cars!
It's frustrating when athletes want sensationalized therapies (cupping, dry needling, compression sleeves etc) but frown upon suggested load management covering mechanical and lifestyle factors. Social media has played a HUGE role. Key driver - former is sexy/$; latter is mundane
Before the online insults begin directed at new exercisers, realize that they’re already facing many barriers. Let’s try to remain kind and empathetic towards them, so they can cont their fitness journey beyond March. And for HCPs/trainers poking fun, shame on you!
If we address posture, does it ensure relief from pain? Many people experience pain despite having “good posture,” while others with imperfect posture feel no pain. Make it make sense

Doing well in life means: loved ones around, a good meal today, a roof over your head, goals to strive for, access to clean water, a supportive family, staying active, good health, steady employment, and waking up each day. Let's value these basics!

It’s interesting how, over my career, the more I’ve learned, the more uncertain I’ve become about whether my interventions are effective or if other factors are at play. I find solace in knowing I’m still promoting health and preventing chronic MSK conditions tho
As a profession, we need to reconcile our ideals with reality. While it’s nice to see 1 patient per hour, Medicare physical therapy reimbursements are as low now as in 1994 — when the dollar held 52% more value than today.
Nice study in @bjsm_bmj showing that resistance training combined with aerobic activity lowers all-cause mortality by a whopping 47%!!!
This is much higher than when each mode is considered independently. Time to stop w the dichotomous thinking and embrace both modes.
Exploring the BPS model with all of its nuances is captivating. At face value, however, it’s application appears to run antithetical to our current healthcare system, meaning patient-centered care doesn’t coexist w declining reimbursement rates.
The catchphrase “knees over toes” doesn’t represent a panacea for knee pain. More so, it reflects human adaptability n the extremes to which our knees are bolstered via small progressions. Whether or not doing such type of exercises are appropriate, well, is context dependent
Diagnostic labels have become more non-specific over the years (I.e. PFP, Lateral hip pain, RCRSP) because we’re realizing that we’re less certain about specific pathoanatomical causes, and just as importantly realize that words matter, to patients. Less is more
It doesn’t matter how evidence-based your exercise prescription is, if you’re not engaging in top-down pain modulation you will be compromising patient prognosis
Don’t step over gold to pick up silver.
If you’re getting up early after only 5hrs of sleep just so that you could go to the gym or run, go BACK to sleep!
