MAPSE: utilidad diagnóstica y pronóstico. 🧐🫀⁉️

🫀From Healthy Atria to Atrial Failure: Rethinking Atrial Cardiomyopathy (AtCM) from the #HFA_ESC clinical consensus statement:
📍AtCM extends beyond #AF: Electrical, mechanical & structural abnormalities may precede clinically apparent AF or #HeartFailure
📍Diagnosis: Electrical atrial dysfunction (P-wave score ≥1) PLUS ≥1 of: mechanical dysfunction, atrial enlargement or excessive atrial fibrosis
📍Proposed diagnostic thresholds:
- P-wave ≥120 ms (standard ECG) or ≥150 ms (amplified ECG)
- Advanced interatrial block: P-wave ≥120 ms + biphasic morphology in ≥2 inferior leads
- LA reservoir strain <23%
- LAVi >40 mL/m² (>48 mL/m² in women >65 years)
- Atrial fibrosis: thresholds pending validation
📍New P-wave scoring system (0–4): Ranges from normal P-wave (0), prolonged P-wave (1), advanced interatrial block without (2) or with paroxysmal arrhythmia (3), to persistent atrial arrhythmia (4).
📍Atrial failure: End-stage AtCM, characterized by persistent AF or HF with symptoms attributable to atrial disease.
📍Early recognition matters! Risk factor & comorbidity management may help prevent or delay progression to atrial failure.

Lightning Strike in the Chest: The Deadly Disguise of Aortic Dissection
————————————————-
This classic presentation is aortic dissection until proven otherwise. A 62-year-old with poorly controlled hypertension develops instantaneous, maximal chest pain that tears straight through to the back—the textbook “lightning-bolt” quality of an intimal tear. The blood-pressure mismatch (right arm 190/105 vs left 155/90) and weaker left radial pulse scream for an acute aortic syndrome involving the arch or left subclavian origin.
Aortic dissection is a true medical emergency. Hypertension drives the intimal tear; once the false lumen forms, it can extend proximally into the coronary ostia or aortic root (causing tamponade or severe aortic regurgitation) or distally down the descending aorta, compromising visceral or limb arteries. Mortality climbs roughly 1 % per hour in the first 48 hours if untreated, which is why rapid recognition beats any lab value.
If the patient remains hemodynamically stable, the single best next imaging study is urgent CT angiography of the chest (often extended through the abdomen and pelvis). CTA provides near-100 % sensitivity for the intimal flap, false lumen, and entry tear while mapping the full extent for surgical or endovascular planning. Bedside transthoracic echocardiography can quickly look for pericardial effusion or aortic regurgitation, but it is not definitive. Transesophageal echo is excellent in the unstable or intubated patient, yet CTA remains first-line when the patient can tolerate the scanner.
Immediate management while awaiting imaging includes aggressive blood-pressure control (usually IV beta-blocker first, then vasodilator to keep systolic pressure 100–120 mm Hg) and pain relief. Antiplatelet agents and thrombolytics are absolutely contraindicated until dissection is excluded. Type A (ascending) dissections generally require emergency surgical repair; type B may be managed medically or with TEVAR depending on complications.
The chest radiograph in this image may already hint at a widened mediastinum, but that finding is neither sensitive nor specific. Only CTA (or equivalent definitive imaging) confirms the diagnosis and guides life-saving intervention. In the ER, the combination of abrupt tearing pain plus pulse or pressure deficit should trigger the dissection protocol every single time.

ECG From Scratch #51: Anterior MI and R-Wave Progression
One of the most important ECG clues to a previous anterior myocardial infarction is loss of normal R-wave progression across V1-V6.
Normally, the R wave becomes progressively taller as you move from V1 toward V6, with the transition from predominantly S to predominantly R occurring around V3-V4.
In an anterior MI, myocardial necrosis can disrupt the normal electrical forces, producing:
- Poor or absent R-wave progression
- Pathological Q waves in the anterior precordial leads
- Persistent QS complexes in some leads
The pattern may help localize the infarct, but poor R-wave progression is not specific for MI.
Other causes include:
🟡Incorrect precordial lead placement
🔵 LVH
⚫LBBB
⚪WPW
🔴Chronic lung disease
So don't read poor R-wave progression as automatically meaning anterior MI.
And remember: an ECG showing Q waves or poor R-wave progression may indicate prior infarction, but it cannot by itself establish an acute MI. Acute coronary syndrome requires clinical context and appropriate ECG/troponin assessment. Current ACS guidelines emphasize obtaining and interpreting the 12-lead ECG rapidly and repeating ECGs when the initial tracing is nondiagnostic but suspicion remains high.

🥚 Egg yolk is not a “hazardous” food for the heart.
AHA and ESC guidance does not recommend blanket avoidance of egg yolk. The bigger picture matters ; overall dietary pattern, particularly saturated fat intake and individual cardiovascular risk.

Aspirin withdrawal is no longer a one size fits all decision.
The timing and choice of P2Y12 inhibitor matter, with ticagrelor or clopidogrel monotherapy considered in different clinical scenarios.
A simple flowchart, but an important reminder that antiplatelet therapy should always be tailored to the patient’s ischaemic and bleeding risk.

Echo From Scratch | Post #31
Apical Two-Chamber View
From the apical four-chamber view, rotate the transducer counterclockwise by about 60° to obtain the apical two-chamber view.
This view focuses on the:
◼️Left ventricle
◼️Mitral valve
◼️Left atrium
Which LV walls are seen?
The two-chamber view shows:
➡️ Anterior wall: right side of the screen
➡️ Inferolateral wall: left side of the screen
Fine adjustments may be needed to visualize the anterior wall clearly because adjacent lung tissue can interfere with the image.
How do you know you've rotated correctly?
A useful technique is to angle posteriorly until both papillary muscles are seen symmetrically.
Then angle slightly anteriorly so that neither papillary muscle is seen in its long axis.
This helps confirm that the imaging plane is correctly oriented.
An important pitfall
The anterior mitral leaflet is seen relatively en face in this view.
Therefore, its apparent closure angle relative to the mitral annulus can be misleading.
Don't interpret leaflet geometry from this plane alone.
Other structures
◻️The left atrial appendage may be visualized adjacent to the anterior LV wall.
◻️By angulating posteriorly and rotating counterclockwise from the A2C position, the descending thoracic aorta can also be brought into a long-axis view.
The key concept
⭕ A4C shows the septal and lateral walls.
⭕ A2C adds the anterior and inferolateral walls.
Together, these views provide complementary information about LV regional structure and function.
