Thank you to @ECUHealthNC for hosting the second annual Men’s Health Fair and Car Show this upcoming weekend.
As a urologist by trade, I understand how important screening is for maintaining good health and ensuring early detection of a growing cancer.

Hospitals could require participation in the same plans as the hospital does as part of credentialing to practice in the facility

Congratulations to our next Chairman of the Board of Governors of the Federal Reserve System, Kevin Warsh who was confirmed today by the U.S. Senate. I have known Kevin for most of his adult life. I first met him as a student in my international politics class at Stanford University, and it was clear even then that he was exceptional.
Kevin is uniquely suited to serve at this moment. He is battle-tested, principled, and brings the breadth of judgment this role demands. Just as important, he understands how deeply international relations and monetary policy are intertwined - a reality that will only grow more consequential in the years ahead.
He is the right person at the right time. On behalf of his friends and colleagues at the Hoover Institution, we wish him well.

Gorgeous day in OKC so a perfect opportunity for some entrepreneurial young ladies to set up a lemonade stand at the park in my neighborhood.
Of course I stopped to ask how much for a glass and reach for my wallet and as I’m doing so, one of the girls says….
“If you don’t have cash, my mom gave me her @CashApp information.”
Welcome to your 21st Century lemonade stand.

One of the most underappreciated dynamics in healthcare is Parkinson’s Law of Triviality: organizations devote disproportionate time to the issues that matter least.
In behavioral economics, it’s called “complexity avoidance.” In healthcare, it’s usually called “the weekly meeting” or sometimes referred to as “the huddle.”
We’ll spend hours debating badge colors, committee names, logo refreshes, and which department should ‘own’ a metric - while multimillion-dollar structural failures, workforce collapse, and catastrophic billing inefficiencies glide by untouched.
The harder the problem, the faster it gets tabled “for further discussion and research.” The easier the problem, the louder the opinions - suddenly everyone’s an expert.
This is why health systems have immaculate branding guidelines but chaotic revenue cycles. Why they can produce a 200-page “cultural competence” report but can’t reliably staff a night shift. Why they optimize hallway signage faster than clinical throughput. Why they talk about all the community service that they produce yet none is ever actually observed or experienced.
This is why dilettantes are rewarded and brilliance is punished. Healthcare doesn’t suffer from a lack of goobers, gomers, and grifters - it suffers from a gravitational pull toward the trivial.
And nothing accelerates that gravity quite like a conference room full of administrators with laptops.

You're a neurosurgeon covering a small community hospital a few days a month.
Every few months you get put on suspension and have to go through administrative song & dance to get privilges reinstated.
The offense? Not cosinging the occasional stray order or PA note.
You ask them to just send a notification if there are outstanding things to click before putting you on suspension.
The answer: "no, you must log in weekly to check your inbox."
You ask if they could please just email a heads up. You'll be happy to check the boxes, but logging in (which requries various layers of lengthy passwords, dual authentication, waiting for slow servers, etc) just to check when 90% of the time there are no deliquencies is a lot of extra burden for just a few days of call coverage per month.
"Nope," they answer, "log in weekly to check. We will not notify you until you're already on suspension."
How much longer will that hospital have neurosurgical coverage?
Administrator run healthcare is why there is a doctor shortage.

We are assigning two “permanent adversaries” to each faculty member. Other adversarial relationships will be on a rotating basis from year to year.
My focus as a health system leader would be on improving clinical operations to improve cost efficiency & convenience.

At @BalladHealth, we are seeing a material increase in unilateral actions by insurance companies to downcode the claims for care provided to patients by physicians and hospitals. This conduct by insurers is intended solely to increase their profits at the expense of patients and their doctors and hospitals. We intend to make sure every patient is made aware when their insurer does this to them.

Not taking anyone's side here.
Worth mentioning that there definitely has been coding drift and intensity in E/M services in care delivery.

This is fascinating. More than 100 Yale professors have called for an audit of the university's sprawling bureaucracy.
The audit, they say, would "preempt potential federal intervention" and send the message: "Yale prioritizes intellectual vitality over bureaucratic inertia."

Workers in any industry should be able to own the business that employs them.
Doctors & nurses - not just large corporations - should be able to own hospitals,

Fixing the injustice done by the AHA and FAH during the negotiations to pass the ACA is a passion of mine. Stopping doctors from owning hospitals was never about patient care or saving money. It was about reducing competition and putting more money in non-physician owned hospitals. There are countless articles and analyses showing that physician owned hospitals achieve the triple aim of reducing cost, improving health of a community, and improving the patient experience.
I am thankful to Senator Lankford for introducing this critical legislation and including my quote in the press release. Let’s do this for the greater good of our healthcare system and undo the wrongs that were done 15 years ago.
