Systemic failures manifest in the emergency department; without hospital and system wide changes, Canadians will continue to face access block, long waits and adverse outcomes in our emergency departments.
🚨 ED overcrowding is not an emergency department problem. It is a health system failure. The new CAEP position statement argues that hospital access block, not inappropriate ED use, is the primary driver of crowding. Patients waiting for inpatient beds experience worse outcomes, while clinicians face increasing burnout and moral distress. Key messages: ✅ Overcrowding is a patient safety crisis ✅ Access block is the main cause ✅ Boarding harms patients and staff ✅ Hospitals need occupancy targets, flow accountability, and discharge capacity ✅ Governments must expand acute, long-term, community, and mental health care capacity The solution is not "fixing the ED." The solution is fixing patient flow across the entire health system. #EmergencyMedicine #HealthPolicy #PatientSafety #AccessBlock #EDCrowding #CAEP #MedTwitter #FOAMed #CJEM link.springer.com/article/10…

The dangers of not allowing the ED to function as an ED...
On the day that Mr. Sreekumar died waiting for care there were 48 out of 51 ED spaces in that ED blocked by sick patients that should have been admitted to hospital. The ED was non-functional. Govt knows the root cause, and has done nothing This review will change NOTHING!

3 things needed to address emergency overcrowding and delays -improved home care and other supports to get people home -increased hospital inpatient capacity -increased long term care More quick care clinics will not solve the problem

Everyone in ER at some point gives up. You can't forever bail water. It's difficult to know if any Ontario government has ever understood the challenges in the ER. We won't have Dr. Drummond forever and I wonder what doctors will correct the record after him. @alandrummond2

Accountability needs to be a top down priority in any successful healthcare organization. Without those at the top having deliverables and consequences, we will simply keep offloading the problem down the chain to the people on the front lines
Unfortunately the author misses the mark on accountability and speaks as an administrator. He states that holding physicians accountable solves northern zone issues. Physicians are already accountable. CAEP is calling for administrators, managers and politicians to have the same level accountability of those on the front line. @raghu_venugopal @alandrummond2 @NightShiftMD @CAEP_Docs

And that was before all the stresses in our system that the pandemic has either created or revealed…

In 2016, Canada's emergency physicians identified a potential shortfall of trained emergency physicians of 1500 by 2025 if things weren't immediately done to improve training and practice. The report was sent to every government; nothing was done. Reap what you sow.

Grateful to be included in this important conversation with the @CMA_Docs. Summer ER closures aren’t inevitable—they reflect deeper system challenges that deserve attention. Thanks for highlighting this issue. cma.ca/about-us/news/system-…

Could you imagine trying to do your job with 75% less resources and capacity? Because that's what our colleagues in Alberta are dealing with right now.
Hey AB: Our functional ED capacity in the Edmonton area has been devastated We have NEVER had overcrowding in our EDs that looks like this in our summer (typically we see admitted volumes decrease in the summer) > 6mnths since Mr. Sreekumar's tragic death, NOTHING is better 1/2

Emergency department overcrowding has become a nationwide patient safety concern, and the causes extend far beyond the emergency room itself. Dr. Michael Herman, Vice Chair of CAEP’s Public Affairs Committee, joined Global News to discuss some of the underlying factors contributing to overcrowding and the need for system-wide solutions. Read CAEP’s position statement on ED overcrowding: caep.ca/position-statements/…

Thank you @KaylaFurlong for guiding the discussion on the root causes of ED overcrowding and highlighting for people from coast to coast where the problems, and solutions, need to lie!

A great interview from @KaylaFurlong about the root causes of ED waits, and the downstream effects on all our patients, and especially our older and most vulnerable population. Strong advocacy from a great Newfoundland leader in the geriatric emergency space!

Correct solutions to the ED Wait Times Problem
We can fix our hospitals. Waiting 44hrs for an inpatient bed shouldn't become the norm. To reduce ER wait-times we must address outflow - enough hospital beds on inpatient units, long-term care spaces for those who need one, supportive housing, and access to quality home care.

Happy @picardonhealth summarized what @CAEP_Docs have been talking about since the CIHI report came out: the problems in the ER are a result of structural problems and access block, and solutions will require system-wide changes, not just another urgent care centre
