There's one thing London doctors get less of than anyone else on the ward: help
with the London cost of living.
Other NHS staff can get up to £8,746 a year. Doctors get £2,162, frozen since 2005. They deserve theirs. So do we.

Just to showcase how far down the rabbit hole some of the Royal Colleges (in this case @RCoANews), along with @NHSE_WTE @NHSEngland @gmcuk et al have all gone- on the hubris of idealogy.
Myth 1:
There isn't enough doctors to do the job.
Fact:
In the context of anaethesia?
The competition ratio for Anaesthetics CT1 in the NHS is 10.54:1 for the 2026 Round 1 recruitment cycle (5,766 applications for 547 posts)
AFTER removing those pesky foreigners aka IMGs (as far as I am aware)
Myth 2:
There isn't enough money to hire doctors
Fact:
Look at recent job advert for a Physicians Assistant (Anaesthesia)
Salary & Pay Scale: NHS Agenda for Change Band 7 (£55,524 - £62,652 per annum)
Above doesn't factor in cost of supervision via a Senior Consultant
Riddle me this.
What is the feasible reason to have these roles when you have trained doctors (NOT foreigners)- done at cost to tax payer to do the role?
It makes no clinical, financial or policy sense.
Yet?
Here we are.
Pontificating why NHS is non-productive or has safety issues - wherever you look.
I repeat to all.
Do NOT do to others you wouldn't want to do to your loved ones.
@RoyLilley @JanetEastham @lengreview @KamranAbbasi @DrNatBroughton @YvetteCooperMP
THIS is the question.
Let me take it one step further.
Your most loved one is due to have surgery.
You have a choice as to who 'puts them to sleep for the procedure'- and then 'wakes them up'.
1. It's either a trained Anaesthetist aka a Doctor
2. Or a Physician Assistant in Anaesthesia aka Not a Doctor
The answer to me is obvious.
Not in bazillion years should it be number 2.
If you @NHSEngland @NHSE_WTE actually believe in 'Informed Choice'?
Inform them properly- not dupe them with made up titles.
What a two tier system we have descended to.
I repeat- do NOT do to others what you won't do to your loved ones.

Calling @RCoANews @RCoAPresident @RCPhysicians @RCSnews
Since when has it been ok for physician assistants to run unsupervised pre- assessment clinics for high risk patients having MAJOR surgery?
Are you going to step in here @mabonapgwynfor @WGHealthandCare ?

£2,162 a year. That was doctors' London allowance in 2005. It still is today. Rent is up 128%. Childcare is up 133%. The allowance? 0%. London needs doctors who can afford to live here.

We need to be crystal clear
They hold no qualifications in medicine and have less medical knowledge than an F1. Yet they are lobbying to be allowed to work at the level of a Senior Doctor
This is utter lunacy and should not be entertained by any thinking person

Yes places are still contravening @RCoANews National guidelines .
For example the 6 AAs in Sheffield are still doing cardiac against RCOA recommendations .
You’d think that the leader of AAs would know about this stuff.

People talk about 'Top Down' healthcare management structures, with back room seniors managing front line healthcare workers.
Rather than call this 'top down', why not call it 'Back to Front', which, in my opinion, is the reality.

Alternatively 'Do the most extensively trained, highly qualified, expert staff, doing a job that nobody else can do, that carries levels of responsibility that dwarves any other healthcare profession, deserve to receive continued real-terms pay cuts year on year?'

We’ve been through this before.
People can expand these old threads and see for themselves.
The PAA ‘profession’ comes first before patient safety.
IMO ALL PAAs should be replaced by a real Dr - who will become a consultant one day.
Redeployment is a thing.

The answer to a shortage of Anaesthetists is ………
Invest in the training of Anaesthetists so that we have more of them
Problem solved .
Why should we ‘rely’ on undertrained fake anaesthetists ?

This government is aiming harm the public .
Why else would they post this job ad for a physician assistant with no experience in mental health to manage Pts with severe mental illness ?

We don’t begrudge our hard working NHS colleagues their London Weighting allowance.
But London is no cheaper for doctors.
Doctors deserve parity to mitigate the higher cost of living and working in the capital.

@RoyalFreeNHS A waste of time , and even at band 7 a waste of money .
This adds to the anaesthesia consultant shortage and is also unsafe without 1:1 supervision throughout.

It’s not “role substitution”, they’re “not practicing medicine” but somehow PAs/ACPs need supervision by Doctors in order to do their jobs & for it to be legally sound?
As AHPs, they didn’t need Dr supervision, if AP is an extension of base role, why is Dr supervision needed?

A very important point re a system which supposedly can’t offer supervision for highly medically trained people (resident doctors) but somehow claims it can safely supervise medically unqualified & untrained people.
Also why is a Dr supervising if it isn’t medical practice?

*The document on 'draft' protocols for Physician Assistants as led by @lengreview has got 2 fundamental flaws in it*
Keeping aside the debate about 'should they see undifferentiated patients' (They shouldn't- for clarity in my opinion)- there are 2 issues which clashes with the basic tenets of @RCPhysicians as they await 'feedback' from Members & Fellows.
1. If a group of individuals -who are anonymous -and have no declared conflict of interest- can create a national document based on which jobs can be created? What indeed is the point of the RCP- whose fundamental job it is to develop standards- as they do in many areas.
The leadership on this MUST come from the Royal Colleges- who should be the standard bearers for supervisors- in this case- Consultants. (H/T @Anisocyte)
2. The @RCPhysicians have produced an admirable piece of work around Resident Doctors- in the hope of aligning with the 'Medical Educative Training Review' as led by Dame @DacreJane.
The METR will struggle regards time needed to supervise/support the Residents- where exactly will it get time to supervise others- at a higher level- as suggested by this document.
The maths don't math.
The RCP should reject the protocols on ground of:
Lack of transparency
Lack of resources
Lack of safety considerations- as highlighted in @lengreview
