🎗️ What Really Changes in Breast Oncology After ESMO 2026?
@myESMO - European Society for Medical Oncology Congress 2026 has delivered major updates across early and metastatic breast cancer, from the six-year NATALEE survival analysis to a rapidly expanding generation of antibody-drug conjugates.
📊 Key findings shaping the field:
🔹 NATALEE: Six-year OS of 91.9% vs 89.7%, strengthening long-term evidence for adjuvant ribociclib.
🔹 HORIZON-Breast06: Median PFS of 16.4 vs 6.8 months with trastuzumab rezetecan in HER2-low advanced disease.
🔹 PANKU-Breast01: A 59% reduction in the hazard of progression or death with the bispecific ADC izalontamab brengitecan.
🔹 OptiTROP-Breast01: Sustained OS benefit with sacituzumab tirumotecan in previously treated metastatic TNBC.
🔹 KEYNOTE-B49 and postMONARCH: Important reminders that improvements in disease control do not necessarily establish an overall survival advantage.
But the next challenge is larger than identifying another effective treatment.
How do we select the right patient, choose the right ADC, sequence therapies after resistance, and balance benefit against toxicity?
In our new OncoDaily Breast editorial, we examine which findings strengthen current practice, which remain investigational, and what oncologists should take away from Madrid.

Oncologists Discuss NATALEE Trial
The six-year #NATALEE trial results presented at #ESMO2026 showed an overall survival benefit and sustained improvements in invasive disease-free survival with adjuvant ribociclib plus endocrine therapy in patients with HR+/HER2− early breast cancer.

🎗️ Does continuing CDK4/6 pathway inhibition after progression translate into longer survival? The final postMONARCH result is encouraging, but not definitive.
At ESMO 2026, abemaciclib + fulvestrant produced median OS of 32.1 months versus 24.6 months with fulvestrant alone after progression on prior CDK4/6 inhibitor + endocrine therapy.
That is a 7.5-month numerical difference, with an HR of 0.80.
However, the final OS endpoint did not reach statistical significance.
Updated PFS continued to favor abemaciclib, supporting the biological concept that progression on one CDK4/6 inhibitor does not necessarily mean complete loss of CDK4/6 dependence.
With oral SERDs, PI3K/AKT pathway inhibitors and ADCs now competing in the same setting, the next question is increasingly about which patients retain enough CDK4 dependence to benefit from continued cell-cycle targeting.

🎗️KEYNOTE-B49 is a positive PFS trial, but not a positive survival trial.
At ESMO 2026, pembrolizumab + chemotherapy reduced the hazard of progression or death by 26% in PD-L1 CPS ≥1 HR+/HER2− advanced breast cancer after endocrine-based therapy.
Median PFS was 6.7 vs 6.0 months (HR 0.74). The signal was stronger in the smaller CPS ≥10 subgroup, where median PFS was 7.5 vs 4.4 months (HR 0.59).
But at the final analysis, median OS was 21.0 vs 22.2 months, with HR 1.04-showing no survival benefit.
The trial therefore raises a more important question than whether immunotherapy can work in HR-positive disease: can we identify the minority of patients who derive durable benefit more precisely than PD-L1 CPS ≥1?

🎗️ Can the cell cycle still be successfully targeted after progression on a CDK4/6 inhibitor? FOURLIGHT-1 suggests it can.
At ESMO 2026, randomized phase II FOURLIGHT-1 showed that the selective CDK4 inhibitor atirmociclib + fulvestrant extended median PFS to 9.4 months versus 6.6 months with investigator’s choice therapy.
That represents a 40% reduction in the hazard of progression or death (HR 0.60).
ORR improved from 11.3% to 19.3%, while clinical benefit rate increased from 46.0% to 61.9%. Importantly, most patients had recently progressed on first-line CDK4/6 inhibitor therapy.
The result challenges the assumption that progression on a conventional CDK4/6 inhibitor necessarily means complete loss of cell-cycle dependence.
FOURLIGHT-1 is still phase II and OS remains immature, but it provides an important proof of concept: selective CDK4 inhibition can retain clinically meaningful activity after CDK4/6 resistance.

🎗️ NATALEE has reached a major milestone: adjuvant ribociclib is now showing an overall survival benefit.
At ESMO 2026, the prespecified 6-year analysis showed that ribociclib + NSAI reduced the risk of death by approximately 19% compared with endocrine therapy alone.
Six-year OS was 91.86% vs 89.73%, an absolute improvement of 2.1 percentage points. Breast cancer-specific mortality was reduced by approximately 29%, while the absolute iDFS benefit widened to 5.3 percentage points at six years.
Perhaps the most important detail: patients had been off ribociclib for a median of about 38 months.
The benefit therefore continues well beyond the planned three years of treatment, strengthening the evidence that finite adjuvant CDK4/6 inhibition can change the long-term course of high-risk HR+/HER2− early breast cancer.
