Dr. Frederick Howard, MD (@fredhow), Assistant Professor in the Department of Medicine, University of Chicago, gave the Susan E. Donelan #InflammatoryBreastCancer Lecture in our Breast Oncology Seminar Series. He gave a talk focusing on artificial intelligence insights into the diagnosis, biology, and precision treatment of #IBC.
A PD-L1 result now decides whether a patient with metastatic TNBC gets pembrolizumab-based therapy or an ADC first, and both paths carry toxicities that need different monitoring.

FourLight-1 presented by @antgiorda at #ESMO26
Atirmociclib + fulvestrant improved median PFS vs investigator’s choice after CDK4/6i progression in HR+/HER2− metastatic breast cancer (9.4 vs 6.6 months; HR 0.60; p=0.0007)

A new signal for targeting CDK4 after CDK4/6 progression. 🧬
In phase 2 #FOURLIGHT1, atirmociclib + fulvestrant reduced the risk of progression or death by 40% versus investigator’s choice in HR+/HER2− advanced breast cancer.
📊 Median PFS: 9.4 vs 6.6 months
The comparator was fulvestrant or everolimus + exemestane. These findings support continued development of selective CDK4 inhibition, with OS still immature and atirmociclib investigational.

🧠 DESTINY-Breast09 | ESMO 2026
T-DXd + pertuzumab vs THP — first-line HER2+ metastatic breast cancer.
Exploratory CNS analysis (n=61):
• CNS response: 51.6% vs 26.7%
• Complete CNS response: 35.5% vs 23.3%
• CNS-PFS: NR vs 9.7 months (HR 0.26)
• Overall PFS (CNS subgroup): 36.5 vs 11.1 months
• Full trial PFS: 40.7 vs 26.9 months
💡 Take-home: Nearly double the brain response and markedly longer CNS control!
⚠️ Small, selected subgroup; exploratory findings. Indicated brain-directed therapy remains important.

🧠 HER2CLIMB-05 vs BRIDGET: When to Add Tucatinib in HER2+ Metastatic Breast Cancer?
🔹 HER2CLIMB-05 | BEFORE progression
HP maintenance + tucatinib
Phase III: PFS 24.9 vs 16.3 months (HR 0.64) ✅
🔹 BRIDGET | AFTER isolated brain progression
SRS → Continue HP + tucatinib
Phase II: Median brain PFS not reached vs historical 5-month benchmark; 18-month brain PFS 59%.
⚠️ Randomized benefit in HER2CLIMB-05; promising single-arm evidence in BRIDGET.

My #esmo26 breast summary: atirmo great data await phase 3; datodxd active in cns; pembro active in pdl1 >10 er+; Chinese “me too” need h2h; same payload no benefit; add tucatinib to delay cns progression; adj cdk os benefit (attn to stats) but giredestrant better tolerability.
